Leonard 2014 Dynasty Trust PO Box 619 New Castle, CO 81647 Via email: joeltleonard@gmail.com Subject: Final Acceptance Letter 1679 County Road 245 New Castle, CO 81647 OWTS Permit #: SEPT-6574 September 24, 2021 Joel, On February 9, 2021, we visited the property at 1679 County Road 245 near New Castle in Garfield County, Colorado to observe the Onsite Wastewater Treatment System (OWTS) installation pursuant to OWTS Permit # SEPT-6574. At the time of our site visit, the contractor had installed a 1,250-gallon two compartment concrete septic tank followed by a 500-gallon concrete pump tank, the new sewer line, the pump lines, and two chamber trenches. The tanks and chambers were installed level, with access to the tanks brought to grade. The two chamber trenches consisted of 21 Infiltrator Quick4 chambers per row, for 42 total chambers and a total STA of 504 ft2. The pump and control box had not been installed at the time of our site visit. On September 9, 2021, we received a letter from High Country Engineering indicating that the pump had been wired, was functioning properly, and was installed in general conformance with the design. The letter also included photographs showing the installation and location of the pump control box and alarms. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Thank you, Edward R. “Ted” White, P.E.
