Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on November 4
and 8, 2021 for the subject property. RMB Excavation installed the system.
The OWTS design is based on 4-bedrooms in the proposed residence plus 1-future bedroom for a total
of 5-bedrooms. An average daily wastewater flow of 600 GPD was used.
The 4-inch diameter SDR-35 sewer line exiting the residence has a double-sweep clean out and a
minimum 2% grade to the septic tank.
The system installation included a 1500-gallon, two-compartment poly Infiltrator® septic tank with an
effluent filter on the outlet tee. This filter must be cleaned annually, or as needed. The inlet
penetration in the septic tank was originally drilled too low. Repairs were made to the penetration in a
water-tight fashion by the excavator.
Effluent gravity flows to a distribution box, and then to four gravelless chamber trenches. The distribution
box was accessible from grade and flow equalizers were installed on each outlet pipe in the distribution
box.
Each trench consists of 15 ‘Quick 4’ Standard Plus Infiltrator® chambers for a total of 720 square feet of
infiltrative area. There was at least 4-feet of undisturbed soil between each trench. The original
installation had the pipe coming into the lower knock-out of the end plate. The trenches were dug slightly
lower and new endplates installed with pipes entering the upper knock-out. Inspection ports were
installed at the beginning and end of each trench. Ports may be cut to grade and placed in sprinkler
boxes for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent of the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the report and design as
submitted by CBO, Inc. dated February 23, 2021. Changes to the design and permit must be
approved by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve a new four-bedroom residence (with a potential future 5th bedroom). The
system will consist of a new, two-compartment 1,500-gallon septic tank set level with access to
grade with an effluent filter in the outlet tee of the tank. Effluent will gravity flow to a distribution
box set level with access to grade distributing effluent to 720 ft 2 of STA. The STA will consist of four
trenches of chambers, each trench with 15 'Quick 4' Standard Infiltrator chambers for a total of 60
Quick 4' chambers (720 ft2). Inspection ports are to be installed at each end of each trench.
Maintain all setbacks and elevations as designed.
