3.2 Grand Vallev Fire Protection District
Roles of the Grand Valley Fire Protection District include:. Emergency Medical Services, Ambulatory Services, Fire Suppression,
Hazardous Materials Mitigation Fire Prevention, Rescue, Training and Public
Education, and other emergency services including ambulance and wild fire
responses.. Provides first response medical services for all injured or ill Company,
contractor, subcontractor, or vendor employees and for site visitors.. Transports injured or ill personnel by ambulance to medical facilities
from Site or related property.. Dispatched by the Garfield County Emergency Communications
Authority and a member of a multi-county mutualaid agreement, aiding
other departments both locally and within the seven member counties.. There are three locations for the Grand Valley Fire Protection District in
Parachute, and they are:o 124 Stone Quarry Roado 200 Grand Valley Way¡ 5797 County Road 309 (unstaffed)
Garfield County Dispatch shall dispatch out to Grand Valley Fire
Protection District and when dispatched, the closest station that is

3.5 St. Marv's CareFlioht Helicooter
ln case of a life threatening situation requiring immediate
medicalattention, the responding EMT or Paramedic will
determine if CareFlight Helicopter Services are necessary.
8.2.4.2 Non-Extinguishable Fire. Report the fire by activating the nearest fire alarm and contacting the
superuisor on duty.. Call 911 and give all needed information, referencing the emergency
numbers listed herein.. Conduct an emergency shut down and evacuate the area.
lf a potential wildfire breaks out, the most important thing is accountability. First
notify someone of the fire. Second, callthe Garfield County 911 Dispatch
immediately. The sooner 911 is dispatched, the quicker the response time.
All fires on federal lands require immediate notification to applicable Ursa
personnel. lf the fire cannot be put out by the fire extinguisher in the incipient
stage, it is time to evacuate the area immediately.
10.2 Reportinq Requirements
Spills or leaks that can be contained wholly on-site or does not represent a
reportable quantity (RQ) value as per 40 CFR 117 should be reported to
the appropriate Ursa representative.
Spills or leaks that meet or exceed RQ values as per 40 CFR 1 17 will result
in the contact of the following agencies:

Hoty Cross Energy has existing power facilities located on or near the above mentioned project.
These existing facilities have adequate capacity to provide electric power to the developmeñt,
subject to the tariffs, rules and regulat¡ons on file. Any power line enlargements, relocations,
and new extensions necessary to deliver adequate power to and within the development will be
undertaken by Holy Cross Energy upon completion of appropriate contractual agreements and
subject to necessary governmental approvals.

hereby requests permission and authority from the Board of County Commissioners to construct a driveway approach (es)
on the right-of-way off of County Road. 303. 25' +/- North oî 134 CR 303, located on the Nofth side of road for the
purpose ofobtaining access to property.
Applicant submits herewith for the consideration and approval of the Board of County Commissioners, a sketch of the
proposed installation showing all the neÇessary specification detail including:
l. Frontage of lot alongroad.
2. Distance &om centerline of road to property line.
3. Number of driveways requested
4. Width of proposed driveways and angle of approach.
5. Distance from driveway to road intersection, if any.
6. Size and shape of area separating driveways if more than onE approach.
7. Setback distance of building(s) and other structure improvements.
8. No unloading of equipment on coi¡nty road, any damage causedto county road will be repaired at subdivision
expense.
9. Responsible for two years from the date of completion.
General Provisions
l) The applicant represents all parties in interest, årnd affrms that the driveway approach (es) is to be constructed by
him for the bona fide purpose of secwing access to his property and not for the purpose of doing business or
servicing vehicles on the road right of way.
2) The applicant shatl fumish all labor and materials, perform all work, and pay all costs in connection with the
construction of the driveway(s). All work shall be completed within thirty (30) days of thepermit date.
3) The type of construction shall be as designated and/or approved by the Board of County Commissioners or their
representative and all materials used shall be of satisfactory quality and subject to inspection and approval ofthe
Board of County Commissioners or their representative.
4) The traveling publie shall be protected during the installation with proper warning signs and signals and the Board
of County Commissioners and their duly appointed agents and employee shall be held harmless against any action
for personal iqiucy or property damage sustained by any reason of the exercise of the Permit,
5) The Applicant shali assume responsibility for the removal or clearance of snow, ice, or sleet upon any portion of
the driveway approach (es) even though deposited on the driveway(s) in the course of the County sno'w removal
operations.
6) In the event it becomes necessary to remove any right-of-way fence, the posts on either side of the entrance shall
be surely braced before the fence is cut to prevent any slacking of the remaining fence and all posts and wire
removed shall be tumed over to the District Road Supervisor of the Boa¡d of County Comm:issioners.
7) No revisions or additions shall be made to the driveway(s) or its appurtenances on the right-of-way without
written permission of the Board of County Commissioners.
8) Provisions and specifications outlined herein shall apply on all roads under the jurisdiction of the Board of County
Commissioners of Garfield County, Colorado, and the Specifications, set forth on the attached hereof and
incorporated herein as conditions hereof.
9) Final inspection of driveway will be required upon completion ¡nd must be approved by person ÍssuÍng
permit or repre$entatíve of persou issuing permit.
The inspecdon and sign off must be done prior to any CO from the Buílding and Planning
Department being issued.

Specifïc?tions
1. Â driveway approacfr is understood to be that portion of thc counly road right-of way between the
pavement edge and the property line that is designed and used for the interchange of traffic
between the roadway and abutting property.
2. At any intersection, a driveway shall be rest¡icted for a suf{icient distance from the intersection to
preserve the normal and safe movement of trafÍic. (It is recommended for rural residence
entrances that a minimum intersection clearance of 50 feet be provided and for rural commercial
entrances a minimum of 100 feet be provided.)
3. All enlrances and exits shall be so located and constructed that vehicles approaching or using
them will be able to obtain adequate sight distance in both directions along the county road in
order to ûuÌneuver safely and without interfering with county road trafüc.
4. The Applicant shall not be permitted to erect any sign or display material, either fixed or
movablg on or extending over any portion of the county road right-of-way.
5. Generally, no more than one approach shall be allowed any parcel or property the frontage of
which is less than one hundrcd (100) feet. Additional entrances or exits für parcels having a
frontage in excess of one hundred 100) feet shall be permitted only after showing of actual
convenience and necessity.
6. All driveways shall be so located that the flared portion adjacent to the traveled way will not
encroach upon adjoining property.
7. No commercial driveway shall have a width greater than thírty (30) feet measured at right angles
To the centerline of the driveway except as increåsed by permissible radii. No noncommercial
driveway shall have a width greater than twonty (20) feet measured at right angles to the
centerline of the driveway, except as increased by permissible radii.
8, The axis of an approach to the road may be at a right angle to the centerline of the county road
and ofany angle beTween niuety (90) degrees and sixty (60) degrees but shall not be less than
sixty (60) degrees. Adjustment will be made according to the type of traffic to be served and othe¡
physical conditions.
9. The construction of parking or servicing areas on the county road right-oÊway is specifically
prohibited. Commercial establishments for customer vehicles should provide off-the-road parking
facilities.
10. The grade of entrance and exit shall slope downward and away from the road surface at the same
râte as the normal shoulder slope and.fur a distance equal to the width of the shoulder but in no
case less than twenty (20) feet from the pavement edge. Approach grades are restricted to not
more than ten percent (10%).
11. All driveways and approaches shall be so constructed that they shall not interfere with the
drainage system of the street or county road. The Applicant will be required to provide, at his
own expense, drainage structures at entrances and exils, which will become an integral parf of the
existing drainage system. The Board of County Commissioners or their representative, prior to
installation, must approve the dimensions and t¡4pes of all drainage structurcs.
Note: This permit shall be made nvail¿ble at the site where and when work is being done. A work
sketch or drawing of the proposed driveway(s) must accompany application No permit will be
issued without drawing, blueprintn or sketch.

Introduction
This site specific SPCC/Containment Plan (Plan) provides the pertinent information with regards
to the environmental and equipment information associated with the respective facility, as required
to ensure procedures and protocol implemented by Ursa to meet all applicable federal regulations
included in Spill Prevention Control and Countermeasure (SPCC) as described in CFR 40 $112.
This site specific plan also contains the relevant information required to meet all state and local
containment-related requirements. The protocol and procedures as provided in Ursa's Field Wide
SPCC/Containment Plan will be applied to the respective facility.
Facility Purpose
The Watson Ranch Well Pad is an existing facility which facilitates the extraction of natural gas
for use in the surrounding area via pipeline infrastructure. Produced water and condensate are
considered by-products ofthe natural gas extraction process and are stored at the facility until their
respective final use is determined. Produced water may be recycled or treated at awater treatment
facility, while condensate can be utilized as an additive to fuel refining processes. A facility detail
summary is provided in Table l. It should be noted that the well pad currently has working
wellheads, flow lines, separators, condensate tanks, and produced water tanks which are subject to
the SPCC regulations.
Please see the Tables and Map included in the following portion of this Plan for additional
information pertaining to the facility.
In the event of a spill please contact the Incident Commander or Alternate Incident Commander
If there is a serious threat to life please call 91 1.
Directions to the Facility
From the town of Parachute, travel south towards Battlement Mesa (CR 215). Continue onto CR
300. There will be a slight left onto N. Battlement Parkway, travel 1.5 miles and turn right onto W
Battlement Parkway. Turn right onto Stone Quany Road and travel 0.75 miles south. Turn left
(east) onto Gardner Lane and travel 0.3 miles east and turn onto the Watson Ranch B pad access
road.

As to your quest¡on regarding trucking of fluids and the details in the submittal: the Project Descr5iption/Narrative has a
long paragraph that details the where, when and how of fluid transport to Watson B. One thing has been modified. The
narrative currently contains the following:
Ultimately Ursa hopes to connect all of their production facilities to injection well(s) via pipeline. ln the interim,
the B&V, Stierberger, Richardson, and Tompkins pads will deliver water via trucks using the Garfield County
Road and Bridge approved haul route. A full traffic impacts analysis has been provided as part of this
submittal.
Richardson is now referred to as Monument Ridge B. Since the original submittal, Ursa has determined that Monument
Ridge B will not require trucking, fluids from that location will be piped.
I will be in the office until noon today and then I will be out until next Wednesday.

This update is provided to you given the concerns expressed by some homeowners in Battlement Mesa, and to keep you
informed with information that may affect the GARCO Watson B UIC permit. As of this morning, Donald and Barbara
Allred have withdrawn their protest (see below from J. Rouse COGCC). This leaves Daniel and Mark Gardner as the only
protestors with standing regarding the COGCC permit. On Friday, we provided information to Ursa legal counsel that in
fact both of these individuals were notified of the pending UIC well in December 2O14, as part of the Form 2A {aka pad
permit) Pre-application and subsequent Completeness Determination notifications (per J. Doose, Ursa Landman).
As information is made available we will keep you informed so you can brief the Commissioners and planning dept. as
appropriate. Ursa wíll continue to work with local concerned citizens, Garfield County, and appropriate state agencies
to ensure that our development activities are conducted in compliance with regulations, and with regard for local
citizens that mây be affected by Ursa's operations.

We wish to thank you for informing us of the requirements that are needed to be considered for a possible hearing about

Unfortunately, we are unable, financially to pursue this matter any further.
Of course our concerns will not change, as we live with them each and everyday.
Our eight acres are surrounded on three sides, by the URSA CO.
ie:
1. Entrance into three drilling pads--Yeater, Watson A, & Watson B.
2. Watson B pad and entrance completely across back of our property, along with their pipeline.
3. Pipeline from back corner of our property, three quarters of the way to the county road, which is the front of said
property.
As you can see, along with the fact that if we needed to drill for water on the back part of our property, we would not be
allowed to,
due to your rules about being within a close perimeter of drilled gas wells. As it shows on your landowners map, our
home is located
750 feet, from said wells.

Spoke to Steve at length. He seemed appreciative of our discussion and he finished with "I suppose most of this
should be done by email.. just wanted to hear your voice once".
Among other things I explained the several bites at the Apple he will have to provide formal and other
comment.
FYI V/hat took this off the tracks for all the residents was Ursa mailing a letter to all that stated they had 15
days from receipt to protest prior to having submitted a complete application to the state and a week or more
prior to submitting to us. That absurd deadline was the first thing I rectified a couple weeks ago. In addition to
submitting a request on their behalf to have COGCC hear this at a hearing rather than do an administrative
review.

Thank you for the feedback and I appreciate your concern on all counts. I will try to call you this weekend.
As noted in the several emails going back and forth to you and the other residents over the last week from me
and Ursa staff, a concerted effort is being made to hear and address your concerns as well as the concerns of the
other residents. I will pass along what you request to the other meeting participants and ensure it is shared
during the meeting next week. If we can conference you in by phone I would be happy to do so. May depend on
cell signal which can be a problem in that area.
I understand Ursa has also extended invitation for you to meet with them on your side of the divide at your
convenience. The primary COGCC staff you would also want to discuss this with have their office in Denver as
well. They are coming over here to discuss the injection well on the 4th.

quoted in the Alaskø Dispatch News, showed that 1 in 6 injection wells had structural violations. Three such
wells have actually bubbled wastes to the surface.
The concern is compounded by recent findings that injection wells have caused earthquakes. Hundreds
of tremors have been attributed to injection wells around the country. Oklahoma ordered an injection well shut
down after a 4. 1 earthquake earlier this year. Greeley, Colorado experienced 60 earthquakes ín 2Ol4 until an
injection well was shut down and the process was changed.
Cleary the process needs work. The article sited above quotes Stefan Finsterle, a leading hydrogeologist for
Lawrence Berkeley National Laboratory as saying: "There is no certainty in any of this" (being safe) "and
whoever tells you the opposite is not telling you the truth. You have changed the system with pressure and
temperature and fracturing, so you don't know how it will behave." The link to this article is included below.
Should URSA decide to overlook these dangers, will the company be willing to post a bond to compensate
residents and property owners for damages?
We request that this information be presented at the meeting on March 4th, either read or through written means
to the attendees in our absence.
