On May 24, 2022, we visited the residence at 49893 Highway 6 & 24 near Glenwood Springs in
Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT‐7409. At
the time of our site visit, the contractor had installed the new concrete 1,250‐gallon two‐compartment
septic tank in the same location as the removed, failed septic tank. The new tank was installed level,
with access to grade and an effluent filter in the outlet tee of the tank. In our opinion, this system was
installed in general conformance with Garfield County OWTS Regulations. Please submit an As‐built
drawing of the system when available. No further action or review from Garfield County Environmental
Health is required regarding this permit.
