Daniels, III 176 GH Daniels Blvd. Gypsum, CO 81637 Via email: ghdgrowit@aol.com Subject: Final Acceptance Letter Lot 6, Quicksilver Court Subdivision 116 Quicksilver Court Rifle, CO 81650 OWTS Permit #: SEPT-19-5947 August 3, 2022 Jodi, On June 16, 2022, we visited the site of the three-bedroom manufactured home under construction at 116 Quicksilver Court near Rifle in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-19-5947. At the time of our site visit, the owner had installed the new sewer line and cleanout adjacent to the residence, the new concrete 1,000-gallon two-compartment septic tank, and the new effluent line. The septic tank was installed level with access brought to grade. An effluent filter had been installed in the outlet tee of the septic tank. On a previous site visit, we observed the installation of the distribution box and the six gravelless chamber trenches. The distribution box was set level, with access to grade. The trenches were level, with 4-feet between the trenches. The trenches consisted of 22 Infiltrator Quick4 chambers per trench (132 total chambers, 1,584 ft2 total STA). Additionally, inspection ports were present at each end of each trench. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. Please submit an As- built drawing of the system when available. No further action or review from Garfield County Environmental Health is required regarding this permit.
