Alpine Excavation and Land Clearing installed the system. The OWTS design is based on 3-bedrooms. An average daily wastewater flow of 450 GPD will be used. A new 4-inch diameter SDR-35 sewer line was installed from the house to the septic tank with a double- sweep clean out and minimum 2% fall. The system installation included a 1000-gallon, two-compartment Norwesco® poly septic tank with an effluent filter on the outlet tee. This filter must be cleaned annually, or as needed. Risers bring the manhole lids to grade for access. Effluent gravity flows to a distribution box, and then to four gravelless chamber trenches. The distribution box was accessible from grade and flow equalizers were installed on each outlet pipe in the distribution box to assure equal flow to each trench. Each trench consists of 9 ‘ADS’ ARC 36 chambers for a total of 36 chambers and 540 square feet of infiltrative area. There was at least 4-feet of undisturbed soil between each trench. Inspection ports were installed at the beginning and end of each trench. Ports may be cut to grade and placed in sprinkler boxes for access. The OWTS component locations were surveyed prior to installation; therefore, the record drawing locations are accurate. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a new 3-bedroom residence. The system will consist of a new, two- compartment minimum 1,000-gallon septic tank set level with access to grade and an effluent filter in the outlet tee of the tank. Effluent will gravity flow to a distribution box set level with access to grade that will distribute effluent to 528 ft2 of STA. The new STA will consist of 4 gravelless trenches. Each row will consist of 11 Quick4 chambers, for a total of 44 Quick4 chambers. Inspection ports are to be installed at the terminal ends of each trench. Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction, and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection, and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced. At the time of our site visit, the owner had installed the new sewer line and cleanout adjacent to the residence, the new poly 1,000-gallon two-compartment septic tank, the new effluent line, the distribution box, and the four gravelless chamber trenches. The septic tank and distribution box were installed level with access brought to grade on the septic tank only. An effluent filter had been installed in the outlet tee of the septic tank. The trenches were level, with 4-feet between the trenches. The trenches consisted of 10 Infiltrator Arc 36 chambers per trench (40 total chambers, 600 ft 2 total STA). Photos were later sent of inspection ports present at each end of each trench, distribution box risers, effluent filter handle installed within 12 inches of grade, and the sewer line tee inside the septic tank. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. Please submit an As-built drawing of the system when available. No further action or review from Garfield County Environmental Health is required regarding this permit. Chamber Requirements Arc chambers are manufactured from high-density polyethylene with an open bottom, solid top and louvered sidewalls. Sidewall louvers shall be designed to minimize soil intrusion. Chamber shall meet the load rating of H-10 (16,000 lb per axle) with a minimum of 12 inches of cover when tested in accordance with IAPMO PS 63 and installed in accordance with manufacturers installation procedures. Chamber Connection Each chamber shall interlock with an integral articulating joint. Articulating joints shall have a free range of horizontal rotation of 20 degrees, with a maximum of 10 degrees in either direction. Articulating joint shall be constructed by placing the dome with engaging knuckle of the incoming chamber over the post end of the previously-installed chamber. Material Properties Each chamber shall be manufactured from high-density polyethylene as defined and described in IAPMO PS 63. Installation Installation shall be in accordance with ADS installation procedures as well as all state and local health department regulations.
