Zamora Excavating installed the system. The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was used. The minimum required 10-foot property line setback was verified. A new 4-inch diameter Schedule 40 sewer line was tied onto the existing sewer line exiting the residence a minimum 2% grade to the septic tank. The system installation included a 1500-gallon, two-chamber Orenco® M1500P-W septic tank with an Orenco® PF3005 pump in the second compartment of the septic tank. There is a filter in this compartment that must be cleaned annually, or as needed. This septic tank, distributed by SCG Enterprises out of Conifer, Colorado, is new to Western Colorado. SCG Enterprises delivered the septic tank and assisted with start-up of the pumping system by instructing the installer. The floats were set to dose approximately 97 gallons each pump cycle. The control panel for the pump is located within line of sight of the septic tank. A 1.5-inch diameter Schedule 40 pump line was installed from the pump to an Orenco® automatic distributing valve (ADV), model 6402. This pump line has a minimum 1% grade for proper drain back into the tank after each pump cycle. The ADV was placed at a high point in the system in an insulated riser with access from grade. Effluent is pressure dosed through 1.5-inch diameter pipes to two beds, each with 3 rows of 15 ‘Quick 4’ Standard Plus Infiltrator® chambers, for a total of 90 chambers and 1080 square feet of infiltrative area. There was at least 6-feet of undisturbed soil between each bed. Effluent is pressure dosed to 1.5-inch diameter laterals, which were hung with zip ties from the underside of the chambers. Laterals have 5/32- inch diameter orifices facing up, with the exception of the first and last holes facing down for drainage. Orenco® Orifice Shields were placed under each downward-facing orifice. The orifices were placed 4- feet on center. Each lateral ends in a 90 degree ell facing up with a ball valve for flushing. Inspection ports were placed at the beginning and end of each trench. Valves and ports were placed in valve boxes, accessible from grade, for access. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. 2-2-2023 LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The new OWTS will serve an existing 4 bedroom residence.The system will consist of a new,two compartment minimum 1,500 gallon septic tank set level with access to grade and a high head pump in the second compartment of the tank.Effluent will be pressure dosed to an Automatic Distribution Valve ADV)set level with access to grade at the high point of the system that will distribute effluent to 1,080 ft2 of STA.The new STA will consist of two three row gravelless chamber beds.The pump line shall consist of 1.5 inch diameter Schedule 40 PVC.Laterals shall consist of 1.5 inch diameter Schedule 40 PVC and have 5/32 inch diameter orifices facing up,every four feet on center O.C.).The first and last orifices on the lateral shall face down,to allow drainage of the laterals.Laterals shall end in a sweeping ell facing up with a ball valve for flushing.Inspection ports shall be installed at each corner of each bed.Maintain all setbacks and elevations as designed. Contact Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit,the applicant hereby agrees to comply with all laws and regulations related to the zoning,location,construction and erection of the proposed OWTS and associated structures for which this permit is granted.The applicant further agrees that if the above said regulations are not fully complied with in the zoning,location,erection and construction of the above described OWTS and structure,the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability,nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system.Permit issuance will not constitute a certification that the system,the equipment used in the system,or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act,the regulations adopted thereunder,or any terms and conditions of a permit. At the time of our site visits, the contractor had installed the new sewer line, the new cleanout adjacent to the building, a new 1,500-gallon two-compartment septic tank with a pump in the second chamber of the tank, the new pump line, the Automatic Distribution Valve (ADV), and the two three-row gravelless chamber beds. The septic tank and ADV were installed level with access brought to grade. The ADV was also installed at the high point of the system. The chamber beds were level, and consisted of 15 Infiltrator Quick4 chambers per row, for 90 total chambers (1,080 ft2). Additionally, inspection ports were present at each end of each trench. The distribution laterals ended in a sweeping 90-degree bend facing up with a ball valve to allow flushing of the laterals. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. Please provide an As- Built drawing of the system, when available, for our files.
