The property was visited on Thursday, 05/18/2023 by Joe O’Neil, P.E. The scope of work was determined by Aspen Foundation Repair and accepted by the owner prior to the involvement of Criterium-Cona Engineers. DESCRIPTION All descriptions of the structure are based on observation from White Peaks Ln. The front of the structure is assumed to face east for the purpose of this report. The subject structure is a two-story home reportedly built in 2005. The home is wood-framed construction with lapped siding and board and batten exterior. Structure has a complex gable/hip-style, asphalt-shingle roof with gutters and downspouts for drainage. There is an attached two- car garage on the left side of the home. The foundation of the home is a concrete stem wall and an assumed footer. The footer could not be directly observed. No foundation or as- built construction drawings were available for review for this project. No soils testing was available for this report. Dry wall cracks emanate generally from corners of building openings in the rea, right, and front interior walls. Significant cracking has extended into the ceiling. Significant dry wall cracking that is extensive and pervasive throughout the interior of the structure signifies likely foundation settlement. PROJECT SCOPE Structural modifications included in this project is the planned installation of forty-five (45) Supportworks Model 288 push piers to provide stability to the perimeter foundation walls of the main structure and attached garage. Installation of the piers is intended to resist further downward movement of the structure. Confirmation of the original stem wall and footer foundation design must be relayed to the engineer prior to any attempt to lift the structure. No lift will be attempted without authorization by the engineer. No lift will be recommended if foundation design is discovered to be a grade beam and pier construction. Please see enclosed drawing # PRJ23025 – 1, 2 for design details associated with this project.
