RE: Certification Letter for Construction of the Onsite Wastewater Treatment System
(OWTS) on Garfield County Parcel Number 212733300358, 308 Cindy’s Way, Rifle,
Colorado 81650, Garfield County, Permit Number SEPT-09-17-4950.
Dear Ted,
This memo is to certify that the Onsite Wastewater Treatment System permitted under Permit
Number SEPT-09-17-4950 for the above address was constructed per the approved design.
Although I was not continuously present for construction of the system, photos provided by, and
discussions with, the Contractor indicate the approved design was followed. In addition,
observations during construction while visiting the site and my final inspection of the system’s
installed features indicate:
All components to the system were properly installed.
Relocated component locations met requirements of Garfield County’s Regulation 43.
The attached record drawing shows as constructed locations for components of the OWTS.
