I am pleased to offer you this report for work performed at the reference site.
OnMarch 28th,2124,April11 2O24andApril3'd,2024,Mr.JoshuaJohnsonwithElementEnvironmental
performed final clearances at 6064 CR 320, Rifle, CO. The final clearances consist of a visual inspection for dust
and debris and ambient air sampling. The final clearances consisted of 3 containments:
1. 03/2812024, at -6:00 pm, an asbestos spill cleanup was performed on the outside of the house, from
the doorway to the dumpster, and of the dumpster itself.
2. O4/OL/2O24, at -10:00 am, an asbestos spill cleanup was performed in the interior of the house.
3. O4lO3l2O24, at -4:00 pm, an abatement was performed to remove the remaining ACM drywall/plaster
in the lower levelof the house.
Any suspect material that is remaining must be assumed to be ACM material until it is tested.
Mr. Joshua Johnson CDPHE AMS #18401 performed a visual inspection. After the visual inspection was
complete, air samples were taken of each of the work areas. Five samples were taken from the work area
The air samples were analyzed by PCM (Phase Contrast Microscopy) per the NIOSH 7400 method. All samples
must below 0.01 f/cc in order to pass the air clearance. All samples were below the limit amount.
Per Colorado Reg 8 Part B guidelines. The work area is now clear for occupancy or normalwork to
resume.
Any suspect asbestos containing materials (ACM) remaining should be assumed to contain asbestos unless
they are properly tested and analyzed by a CDPHE accredited inspector. All work performed MUST follow
appropriate EPA" CDPHE, OSHA, and local government guidelines to protect the employees, occupants, and the
environment.

1.0- This was an inspection for suspect ACM (Asbestos Containing Material)for a complete
demolition. Element Environmental conducted an inspection for materials that would be
impacted during the demolition. This inspection was a full inspection for demolition of
the home. lf other suspect materials become visible/apparent during demolition or
construction activities, work in that area should be halted and more sampling/testing
must take place, per Colorado regulations. Any suspect materials that were not tested
are to be assumed to contain asbestos.
A limited bulk sampling of from allsuspect materials was conducted in accordance with
Colorado State Requirements. The minimum number of samples per homogenous areas
are as follows:
2.1- Surfacing Materials- <1,00O sq'ft'- Minimum of 3 samples
1,000 sq.ft.- S,(X)Osq.ft.- Minimum of 5 samples

2.3- Thermal Systems lnsulation (TSl)-Minimum of 3 samples
3.0- The purpose of the inspection is to identify materials that are homogenous. A
homogeneous area is defined as one which shares suspect material, texture, color,
location, and/or apparent time of construction. The materials are touched to determine
friability. Friable ACM is material that can be crushed or reduced to powder by hand
pressure. The friable materials are segregated into separate homogenous areas. The size
of the sampling area is determined. Samples are taken according to a random numerical
sequence of the subareas, following guidelines per the EPA Pink Book: Asbestos in
Buildings: Simplified Sampling Scheme for Friable Surfacing Materials.
4,O- Scott Roahrig, a Colorado Department of Public Health & Environment certified building
inspector, certification number: 29245, performed the inspection. The findings were
reviewed by Josh Johnson, a Colorado Department of Public Health & Environment
certified building inspector, certification number: 18401. Samples taken were sent to
CEI Labs in Cary, North Carolina. The samples were analyzed by PLM (Polarized Light
Microscopy) analysis. Per the CDPHE Regulation any sample that is found by a lab to be
trace, or <1% asbestos, is considered to be ACM material. This material would still be a
regulated material by CDPHE. The sample must have an additional "point count"
performed to be considered less than !o/o and not be regulated by the CDPHE. The lab's
documents with analyses and findings are found below'

The areas on the Table of Results in Black did not contain asbestos
The areas on the Table of Results in Red contained asbestos

The drywall was removed from almost all of the house. More than 32 square feet of asbestos
containing materialwas disturbed so this constitutes a Mdior Asbestos Spill.
The following Regulation 8 rules must be followed:
lll.T.1. Spill
ln the event of a spill involving disturbance of suspect materials in an amount greater than the
trigger levels: ll 1.T.L.a.
lf disturbed materials are already known to be or will be assumed to be asbestos containing,
the facility owner, operator or contractor who discovered or created the disturbance must
immediately comply with subsection lll.T.2. and submit to the Division and the facility owner or
operator the form titled "Major Asbestos Spill Notification Form" as specified by the Division.
lll.T.1.b. lf the asbestos content of the disturbed material(s)is unknown, the facility owner,
operator or contractor must immediately contact a Colorado-certified Asbestos Building
lnspector to collect and submit for analysis bulk samples in accordance with subsection lll.A.4.
The inspection report must identify if the amount of ACM disturbed constitutes a major or
minor asbestos spill.
The asbestos building inspector who identifies a major asbestos spill must immediately notify
the facility owner, operator or contractor and within 24 hours, submit to the Division and the
facility owner or operator the form titled "Major Asbestos Spill Notification Form" as specified
by the Division.
lll.T.2. Major Asbestos Spills ln the event of an asbestos spill involving greater than the trigger
levels of ACM, the facility owner, facility operator or contractor must:
lll.T.2.a. Restrict access to the area and post warning signs to prevent entry to the area by
persons other than those necessary to respond to the inciden!
lll.T.2.b. Shut off or temporarily modify the air handling system to prevent the distribution of
asbestos fibers to other areas;

lll.T.2.c. Spill Delineation
lll.T.2.c.(i) lndoor Spill Delineation Unless the entire facility is to be treated as a major asbestos
spill, a Colorado certified Air Monitoring Specialist (AMS) must determine the extent of the spill
area. This may be done using visual examination, air samples, microvacuum dust samples, wipe
samples or a combination thereof. lf visible dust or debris is observed, directly related to or
resulting from the known or assumed ACM which created the major asbestos spill, areas where
it is observed must be included in the abatement of the spill.
lll.T.2.c.(ii) Outdoor Spill Delineation The AMS must identify the extent of any debris and any
surface contaminated by debris generated by the spill. This may be done using visual
examination, microvacuum dust samples, wipe samples or a combination thereof. lf visible dust
or debris is observed, directly related to or resulting from the known or assumed ACM which
created the major asbestos spill, areas where it is observed must be included in the abatement
of the spill.
lll.T.2.c.(iii) Samples must be collected and analyzed quantitatively by TEM using the following
methods:
lll.T.2.c.(iiiXA) Air samples must be collected and analyzed in accordance with the procedures
specified in 40 C.F.R. Part763, Appendix A to Subpart E (EPA 2010) "lnterim Transmission
Electron Microscopy Analytical Methods."
lll.T.2.c.(iiiXB) Microvacuum samples must be collected and analyzed in accordance with the
procedures specified in the ASTM lnternational Method D5755-09 (Reapproved April 2OL4),
"standard Test Method for Microvacuum Sampling and lndirect Analysis of Dust by
Transmission Electron Microscopy for Asbestos Structure Number Surface Loading." 7L
lll.T.2.c.(iii)(C)Wipe samples must be collected and analyzed in accordance with the procedures
specified in the ASTM lnternational Method D5480-19 (January 2OL9), "Standard Test Method
for Wipe Sampling of Surfaces, lndirect Preparation, and Analysis for Asbestos Structure
Number Surface Loading by Transmission Electron Microscopy."
lll.T.2.d. The General Abatement Contractor (GAC) selected to perform the cleanup of the spill
must: lll.T.2.d.(i) Submit notification in accordance with subsection lll.E. (Notifications) or
subsection lll.G. (Permits), whichever is applicable, to the Division for approval. lll.T.2.d (ii)
Using certified Workers and Supervisors, in accordance with Section ll. (Certification
Requirements), construct a containment in accordance with the following mandatory
subsections, which must be followed in the exact sequence shown:
lll.T.2.d.(iiXA) Construct the decontamination unit in accordance with subsection lll.K.
(Decontamination Unit).
lll.T.2.d.(iiXB) lnstall critical barriers in accordance with subsection lll.l. (Critical Barrier
lnstallation)
lll.T.2.d.(iiXC) Comply with the requirements of subsection lll.J. (Air Cleaning and Negative
Pressure Requirements

Ensure items cleaned as part of the spill response are included in the clearance process.
lll.T.2.e. All persons must comply with any other measures, provided in writing by the Division,
which are deemed necessary to protect public health.
These materials are considered non-friable. lf the sealant caulk, seam tape and paint will be rendered friable by
mechanical means, then the removal of it is regulated by the CDPHE. lf not, then the removal of it is regulated by
OSHA. You must consult your landfill to see if they will accept it and how it must be prepared. lt may need to be
segregated from the other materials so that the landfill may accept it. Element Environmental recommends that an
abatement contractor remove these materials to ensure that the workers are trained and that the materials are
handled correctly. All work performed MUST follow all EPA and OSHA regulations to protect the general publiq the
client, and the workers.

vinyl floor tiles can be difficult to analyze via polarized light microscopy (PLM). EPA recommends lhat
all NOBs analyzed by PLM, and found not to contain asbestos, be further analyzed by Transmission
Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is
not covered under NVLAP accreditation. Estimatd rneasurement of uncettainty is available on
reguesf.
