Onsite Wastewater Treatment System (OWTS) Installation Observations

Matt and Julie,
CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on December 16,
2022, March 30 and September 9, 2024 for the subject property. Mountain Works installed the system.
The well location changed from our original design documents. We have approximated the location on
the record drawing. We verified the well is greater than 50-feet from the septic tank and greater than
100-feet from the soil treatment area (STA).
The OWTS design is based on 5-bedrooms. An average daily wastewater flow of 600 GPD was used.
The 4-inch diameter SDR 35 sewer line exiting the residence has a double-sweep clean out and a
minimum 2% grade to the septic tank.
The system installation included a 2000-gallon, two-compartment Valley Precast concrete septic tank
with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump in the second compartment of the
septic tank. There is also a filter in the second compartment of the septic tank. This filter must be
cleaned annually, or as needed.
The septic tank was anchored with Deadmen because of the high seasonal groundwater table. The floats
were set to dose approximately 65 gallons each pump cycle. The control panel for the pump is located
within line of sight of the septic tank. Valley Precast out of Buena Vista performed start-up of the pumping
system. The start- up was conducted with temporary power.
Effluent is pressure dosed through a 1.5-inch diameter pump line to a partially mounded, unlined sand filter.
The sand filter is divided into two beds, each 12’ x 31.5’ with some room between beds for plumbing for a
total approximate length of 65-feet. The excavation for the sand filter began 2.0-foot below native grade
(6206’). A minimum of 3-feet of sand filter material was installed in the over-excavated footprint, resulting in
1-foot of sand filter material extending above native grade. A gradation of the sand media is enclosed.
Effluent is pumped through a 1.5-inch diameter pump line from the pump chamber to an Orenco®
automatic distributing valve (ADV), model 6404. The pump line was placed in a Schedule 40 conduit where
it crosses the driveway. The ADV was placed at a high point of each zone in an insulated riser with access
from grade.
Level, 1.5-inch diameter manifolds connect two laterals on each bed. Laterals are 1.5-inches in diameter
with 5/32-inch diameter orifices facing down, spaced 2-feet on center, installed on the GeoMat™ with
the filter fabric over the laterals. Laterals were placed 2-feet from the edges of the beds leaving 3-feet
between laterals and 2-feet between each section connected by the ADV. Four 39” wide GeoMat™ were
placed on the 12-foot wide bed.
Each 1.5-inch diameter lateral ends in a sweeping ell facing up with a ball valve for flushing. Laterals
were covered by the GeoMat™ separation fabric and at least 1-foot of topsoil. Inspection ports were placed
into the sand filter material on each corner of the beds.
The mound must has a minimum 3:1 slope (horizontal:vertical); therefore, the basal area will extend
approximately 9-feet beyond the sand filter on all sides (with a mound height of approximately 3.0-feet above
native grade). Soils in the sloped perimeter areas of the mound must be well draining and able to support
vegetative growth. This soil must be approved by this office prior to installation. The mound must be
crowned in a manner to promote drainage off the STA.
On September 9, 2024 we found the pumping system to be in the OFF position and the alarm light on. We
turned the pump to the ON position and cycled through the ADV. We worked with the electrician and after
some connecting/disconnecting in the panel, the audible alarm sounded normally. We also tested the
pumping system via the floats and the pump kicked on and off normally, and the alarm sounded when the
high water float was lifted.
Weeds have grown over the STA. We recommend removing the weeds, leveling the STA with some topsoil,
and planting hearty, native grasses this fall. Irrigating the area lightly to establish the grasses is acceptable.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the design as submitted by CBO
Septic Consulting dated March 15, 2022. Changes must be approved by Garfield County Public
Health and the designer prior to construction.
The new OWTS will serve a new 5-bedroom residence and equipment barn. The system will consist
of a new, two-compartment minimum 2,000-gallon septic tank set level with access to grade and a
high-head pump in the second compartment of the tank. Effluent will be pressure-dosed to an
automatic distributing valve (ADV) set level with access to grade at the high point of the system that
will distribute effluent to 750 ft2 of STA. The new STA will consist of two unlined sand filter beds
constructed of 3 feet of secondary sand material. The beds will be partially mounded, with 2 feet
below grade and one foot above grade. The pump and effluent lines will be constructed of 1.5-inch
diameter Schedule 40 PVC. The laterals in each bed will be connected with level manifolds, and the
laterals shall have 5/32-inch diameter orifices facing down spaced every 2 feet on-center (O.C.),
installed on GeoMat placed directly on the sand. Inspection ports are to be installed at each corner
of each bed. Maintain all setbacks and elevations as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.

cycles with no issues.
