RE: Review of OWTS Construction for 80 North Meadow Drive
Dear Ted:
The purpose of this letter is to confirm that Mountain Cross Engineering, Inc. reviewed the
construction of the OWTS for the residence at 80 North Meadow Drive. The construction of the
OWTS was performed by Mr. Keith McLaren with Dirt Devil Excavation, Inc, #970.274.1533.
On Saturday May 11th, 2024 a representative of Mountain Cross Engineering, Inc. reviewed the
profile hole that was excavated and performed a visual and tactile analysis on the soils. Neither
groundwater nor bedrock was encountered. Below the top 4” to 6” of topsoil, the soils were
consistent for the 8 ft. depth with no defined layers. The soils formed a ribbon that would break
between 1” to 2” long. The soil felt gritty. The soil was classified as a Soil Type 3, Sandy Clay
Loam which is consistent with the design assumptions and the conservative classification from the
percolation tests of the original OWTS.

Only one profile hole was excavated, at that time, to keep the existing OWTS operational. Dirt
Devil Excavation excavated the second profile hole within the existing leaching area on July 1,
2024 and verified the absence of groundwater or bedrock. Soils were consistent with the other
profile hole. Neither profile hole had discoloration or efflorescence that would indicate seasonal
fluctuation of groundwater; although the second profile hole had a pocket of discoloration that is
almost certainly from the existing OWTS.

On Thursday July 4th, 2024 a representative of Mountain Cross Engineering, Inc. reviewed the
construction of the OWTS. Pictures of the installed components were taken. The dosing siphon
was tested and functioned as designed. Some chamber trenches were partially backfilled with the
ends exposed. The following are pictures from that review.

Based on that review, some items were deficient and needed to be remedied:
1. The outlets from the distribution box were not level. Also, during the dosing from the siphon,
water would splash out of the sides.
2. The southern row of chambers had a damaged end cap and the inspection port was not installed.
3. The outlet side of the dosing chamber was leaking.
The Contractor was asked to repair these and send pictures. Attached are the pictures that were
sent showing that these items had been addressed:

On August 10th, 2024 a representative of Mountain Cross Engineering, Inc. reviewed the finished
construction of the OWTS and took as-built measurements. All inspection ports were installed
under fiberglass yard boxes set to finished grade as well as the septic-tank lids and the distribution
box except that the dosing tank lid was buried. Pictures of some typical yard boxes and of the
installed components follow. As-built measurements are included in the attached plan sheet.
