understand that the Building Division accepts the Application, along with the plans and speeifieations and other
data submitted by me or on my behalf (submittalsl, based upon my certification as to accura*l. Assuming
completeness of the sub'mittals and approvalof this Application, a Building Permit will be issued granting
perrnission to fie, as Awner, t6 eonstruct the structure{sl and facilities detailed on the submlttals reviewed by
th€ Buiiding Division. ln consideration of the issuance of the Building Permit, I agree that I and my ag€nts wlll
comply with provisions of any federal, state or local law regulating the work and the Garfield eounty Buitding
Code, OWTS regulations and applicable land use regulations {eounty Regulation(s}}. I aeknowledgc that the
Bu|lding Permat may be suspended or revoked, upon notiee from the eounty, if the loeation, eonstruetion or use
of the structure(sl and facility{iesf, described above, are not in eomplianee with eounty Regulation(s) or any

The Building Division will mllect a Plan Rcview fee at time of application submittal- Thc permit fce, as
well as any Road lmpact fees will be collected when thc permit is lssued.
The permit application must be signed by the owncr or by person having written authority from the owner
to ad as their representative.
Building cannot be occupied until a Certificate of Occupancy (CO) is issued. Using th€ building without a
