HomeMy WebLinkAboutGCPH Final Acceptance Letter 11.14.2024Garfield County Public Health Department – working to promote health and prevent disease Public Health Shonna and Daniel Scott 80 North Meadow Drive Rifle, CO 81650 Via email: dscott81647@gmail.com Chris@mountaincross-eng.com Subject: Final Acceptance Letter 80 North Meadow Drive Rifle, CO 81650 OWTS Permit #: SEPT-8353 November 14, 2024 The Onsite Wastewater Treatment System (OWTS) permitted under OWTS Permit No. SEPT-8353 at 80 North Meadow Drive near Rifle was installed and buried without an inspection of the installed system by Garfield County Public Health (GCPH). Therefore, GCPH issued a Notice of Violation to you, the homeowner, on August 12, 2024. The design engineer inspected the system prior to the system components being backfilled and provided a letter certifying that the system was constructed in compliance with the permitted design and an as-built drawing of the constructed system (dated August 16, 2024). These documents provided photographs and locations of the installed OWTS components. On September 27, 2024, GCPH personnel visited the site to observe the installed OWTS components from the ground surface and observe the functionality of the siphon. Prior to our site visit, the contractor had installed a new, 1,000-gallon poly septic tank with associated inlet and outlet tees, an effluent filter in the outlet tee of the tank, a new concrete dosing chamber, the new dosing siphon, effluent pipe, concrete distribution box, and 4 chamber trenches consisting of 17 ARC 36 chambers per trench (68 total chambers, 1,020 ft2 total STA). Inspection ports had been installed at each end of each trench. The dosing chamber access had not been brought to grade. During the initial site visit, the dosing siphon did not appear to be functioning appropriately. The design engineer requested an additional site visit once the issues with the dosing siphon had been addressed. GCPH revisited the site on November 1, 2024. At the time of our final site visit, the dosing chamber access had been brought to grade and a dosing counter had been added to the system. GCPH observed proper functionality of the dosing siphon during this site visit. In our opinion, this system was installed in general conformance with Garfield County OWTS Regulations. No further action or review from Garfield County Environmental Health is required regarding this permit. This letter also resolves the OWTS Notice of Violation issued by Garfield County on August 12, 2024. Thank you, Edward R. “Ted” White, P.E.
