HomeMy WebLinkAboutOWTS Installation Observation 12.12.24Page 14 of 17 TO: Mr. Ted White, Garfield County Env Health / Shania McCain, Env Health Specialist FROM: Rick L Barth, PE, SGM Civil Services DATE: December 12, 2024 RE: Umbrella Commercial Site OWTS Record Affirmation OWTS Permit SEPT-7909 In November of 2024 SGM was contacted by Terralink Structures on behalf of Loose Cannon for the commercial parcel 5169 County Road 154, south of Glenwood Springs. The site was a new construction on a previous light industrial yard for a commercial/business facility and warehouse. The original design report was submitted in the fall of 2022. Minor but understandable alterations have been made to the layout. Plumbing exits from the building warranted (for ease of construction) two separate connections that led to the relocated tank in the parking (as opposed to next to the driveway). The tank is traffic rated and has concrete topping above for load bearing. The Orenco pump system is in the tank and panel attached to the structure. The field is in the targeted location of the test pits, with ever so slight re-orientation just to fit the available flat space better. The final ADV was installed and grades/vegetative cover are per Reg 43 standards of relatively flat and non-irrigated grasses only. These alterations have been shown and were noted in the permit application in January. This memo is to provide our affirmation of that installation. The elements are in the appropriate locations or just with very mild alteration due to construction constraints, but nothing to alter design intent or function. Altitude Septic provided photos of the squirt test for the STA bed and such documentation is included. Please accept this letter as our affirmation of the OWTS installation for the above site associated with the record drawings of Fall 2024, provided by the installer, Altitude Septic. Our field walk affirm this as an accurate representation of the constructed conditions.
