Special Inspection Report – 11/23/24 through 1/10/2025
DATES: November 23rd 2024 through January 10th, 2025

DESCRIPTION OF WORK INSPECTED:
Special inspection to monitor erection and A325 High – Strength bolting of structural steel as required by the structural
engineer, IBC and City of Glenwood Springs Building Department in accordance with the approved specifications and
applicable building code requirements.
11-23-2024
Provide periodic special inspection of field welding performed. During the inspection it was determined that the metal
building erection contractor did not follow the requirements provided earlier in the job to at all times exercise code
compliant storage for electrodes and not perform welding on wet surfaces. It was observed that welding to be performed
that morning was poorly fit up resulting in excessive gaps at weld joints. Also noted that the weldor performing this work
was utilizing welding electrodes that were rusty and held in a container that contained some amount of water. Welding
operator was also observed to be performing welding on wet surfaces as a result of the inclement weather. Weldor was
informed that these circumstances were not compliant, and the welding performed effectively failed inspection. Weldor
then chose not to continue performing this work.

Continued special inspection for field welding repairs. Monitor grinding to completely remove old welds. Observe prep to
correctly apply new welds, monitor replacement welds being performed at gridline A/23-26. It was again noted today that
weldor had the correct electrodes and the correct method of storage for those electrodes. At the close of today's
inspection, this work continues in progress.

Scheduled for inspection of field welding; waited on jobsite for crew - mid a.m., Weldor called in sick. The inspection was
cancelled on-site.
12-13-2024
Continued special inspection for field welding repairs as outlined on 12-11 -2024. Monitor grinding to completely remove
old welds. Observe prep to correctly apply new welds, monitor replacement welds being performed at gridline A/15-18. It
was again noted today that weldor had the correct electrodes and the correct method of storage for those electrodes. At
the close of today's inspection, this work continues in progress.

Special inspection to witness field welding (replacement welds) of work in process to attach tubular steel beam to column
connection clips at upper level and roof as outlined above, gridline A/24-27 (Northeast of building).

Special inspection to witness field welding (replacement welds) of work in process to attach tubular steel beam to column
connection clips at upper level and roof as outlined above, gridline A/15-18 (Northwest of building).
1-3-2025
Provide special inspection of field welding performed on the south side gridline PP to repair defective welding previously
performed in the installation of tube steel exterior awning supports. It was noted today that at one upper and lower
awning attachment tube steel header, was that the tube steel used was too short not allowing for enough bearing onto the
L-angle clips used to attach the header tube onto the adjacent tube steel support columns. A directive from the Structural
Engineer will be required to repair this situation. Required welding at this location was suspended today pending a repair
directive. Welding work completed today appears fully to comply with approved plans and applicable codes.
Today's work was hampered when the ice built-up on the roof above began melting and the result dripped down on the
weld areas to be repaired. No materials on the job site sufficient to mitigate water intrusion could be located. At the close
of today's inspection, this work continues in progress.

Special inspection to witness field welding (replacement welds) of work in process to attach tubular steel beam to column
connection clips at upper level and roof as outlined above, gridline PP/15-18 (Southwest) and 24-27 (Southeast of
building) - Tube Steel header beams supporting awning frame.
1-8-2025
Special inspection for field welding rework of tubular steel awning frame attachment support beams along gridline PP/24-
27 and gridline PP/12-15. This work includes the installation of Structural Engineer prescribed gusset plates installed as a
repair for insufficient bearing of tubular steel headers onto L-angle attachment clips. Observe removal of defective weld
from prior and monitor rewelding as required. Noted weld defects were corrected in my presence. Work today was halted
mid-afternoon when the ice melting on the roof caused water infiltration onto the areas to be welded. Nothing on the job
site was available to us to mitigate this circumstance. Work to complete field welding and installation of gusset plates
rescheduled for tomorrow morning.

Special inspection to witness field welding of work in process to attach tubular steel beam awning support frame
sections to column connection clips with multi-pass flare bevel welds at upper and roof level as outlined above, gridline
vicinity PP/24-27 (Southeast of building).
1-10-2025
Provide special inspection for field welding performed to complete weld repairs to tubular steel awning frame attachment
points on gridline PP/12-15 along with gridlines PP/24-27. It fully appears that the previously noted defective welding
work has been removed and rewelded in a compliant fashion. This work along with the previously performed corrections
to A-325 High strength bolted connections performed on 11-25-2024 makes the building fully compliant for high strength
bolting and the required field welding from the exterior wall sheeting inward from northmost gridline A to southmost
gridline PP and westmost wall line on gridline 3 moving east to eastmost wall line on gridline 36.
NOTE: All field welding of structural steel by certified welders employed by LTJ on behalf of Andover Construction as
required by the structural engineer, owner and local building jurisdiction (City of Glenwood Springs) per approved plans
and specifications. All work was prepared and executed in compliance per AWS D1.1, IBC and AISC 360 Section N.
