Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on September 5,
2024 for the subject property. You, the property owner, installed the system.
The OWTS design is based on 4-bedrooms in the residence and 1-bedroom in the ADU. An average
daily wastewater flow of 675 GPD will be used. Only the ADU had been constructed at the time of the
installation.
The 4-inch sewer line exiting the ADU had a double-sweep clean outs and a minimum 2% grade to the
septic tank. Areas subject to traffic were minimum Schedule 40 strength sewer pipe.
The system installation included a 1500-gallon, two-compartment septic tank with an Orenco® Biotube
Effluent Filter. Effluent gravity flows to a 500-gallon, single-compartment pump chamber with a Zoeller®
pump package. Filters must be cleaned annually, or as needed. The control panel for the pump is
located within line of sight of the septic tank.
Effluent is pumped through a 1.5-inch diameter pump line from the pump chamber to an Orenco®
automatic distributing valve (ADV), model 6605. The ADV was placed at a high point in the system in an
insulated riser with access from grade.
Effluent is pressure dosed through 1.5-inch diameter pipes to five gravelless chamber trenches, each
with 18 ‘Quick 4’ Plus Standard Infiltrator® chambers, for a total of 90 chambers and 1080 square feet of
infiltrative area. There was at least 4-feet of undisturbed soil between each trench.
Effluent is pressure dosed to 1.5-inch diameter laterals, which were hung with zip ties from the underside
of the chambers. Laterals have 5/32-inch diameter orifices facing up, with the exception of the first and
last orifices facing down for drainage. The orifices were placed 3-feet on center. Each lateral ends in a
90 degree ell facing up with a ball valve for flushing. Valves may be placed in a valve box, accessible
from grade, for access. Inspection ports were placed at the beginning and end of each trench. Ports
may be cut to grade and covered with a valve box for access.
The OWTS was generally installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS
in the future. Any additional OWTS construction must be according to the county regulations.

SEPTIC CONSULTING
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions :
This OWTS Permit is approved on the condition of compliance with the updated report and design as
submitted via email by CBO on December 1, 2023. Changes to the design and permit must be
approved by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve a new 4-bedroom residence and 1-bedroom ADU. The new OWTS
components will consist of a new, two-compartment 1,500-gallon septic tank set level with access to
grade and an effluent filter installed in the outlet tee of the tank followed by a SQQ-gallon pump
chamber with a high-head pump in the chamber. Effluent will be pumped to an Automatic
Distributing Valve (ADV) set at the high point of the system that will distribute effluent to 1,080 ft 2 of
STA. The new STA will consist of five gravel less chamber trenches, each trench consisting of 18
Quick4 chambers per trench (90 chambers total). Distribution laterals are to be 1 ½ inch Schedule 40
PVC, with 5/32-inch orifices 3-feet on center and the laterals ending in 90-degree sweeping bends
and flushing valves at the distal end of the laterals. Laterals are to be hung from the underside of the
chambers with zip-ties. The orifices are to be facing upward, except for the first and last orifice
(which shall face downward for drainage purposes). Inspection ports are to be installed at each end
of each trench. Maintain all setbacks and elevations as designed .
To assist in scheduling, please contact Garfield County Public Health (970-945-6614 ext. 8150) to
notify the start of OWTS construction. Contact the designer and Garfield County Public Health at
least 48 hours prior to requesting final inspection of the installed system prior to backfill. NO OWTS
COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR APPROVAL FROM GARFIELD COUNTY PUBLIC
HEALTH AND THE DESIGNER.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above .
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction, and erection of the proposed
OWTS and associated structures for which this permit is granted . The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection,
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void .
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system , or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.

Do:
• Have your system inspected (in general) every three years by a licensed contractor
and have the tank pumped, when necessary, generally every three to five years.
Don't:
• Pour cooking grease or oil down the
sink or toilet.
• Rinse coffee grounds into the sink.
• Pour household chemicals down the
sink or flush them.
Don't:
• Flush non-degradable products or
chemicals, such as feminine hygiene
products, condoms, dental floss,
diapers, cigarette butts, cat litter, paper
towels, pharmaceuticals.
Don't:
• Park or drive on your drainfield. The
weight can damage the drain lines.
• Plant trees or shrubs too close to your
drainfield, roots can grow into your
system and clog it.
Don't
• Concentrate your water use by using
your dishwasher, shower, washing
machine, and toilet at the same time.

your septic system .

• Eliminate or limit the use of a garbage

• Consult a septic service professional to
advise you of the proper distance for
planting trees and shrubs, depending on
your septic tank location.

• Stagger the use of water-generating
appliances. This can be helpful
especially if your system has not been
pumped in a long time.
• Become more water efficient by fixing
plumbing leaks and consider installing
bathroom and kitchen faucet aerators
and water-efficient products.
