Garfield County Public Health (GCPH) was not contacted to inspect the OWTS installation at the residence located
at 3726 County Road 214 near Silt in Garfield County, Colorado pursuant to OWTS Permit # SEPT-8529. We
understand that the OWTS was installed in April of 2021. Since GCPH was not called to perform a final inspection
of the installed system, GCPH issued a Notice of Violation to the homeowner. The system was designed by Kumar
and Associates, Inc. and installation contractor is unknown. We received an as-built drawing of the constructed
system and a letter certifying that the system was constructed in compliance with the permitted design from the
OWTS designer dated December 4, 2025. Photographs of the installed system were also part of the certification
letter and as-built drawing of the installed system.
Photographs depicted a new cleanout and sewer line, an existing concrete 1,000-gallon two-compartment septic tank
with an effluent filter in the outlet tee of the second compartment of the tank, the new effluent line, the new
distribution box, and the Soil Treatment Area (STA). The septic tank and distribution box had access brought to
grade. The STA consists of three gravelless chamber trenches, with each trench consisting of 18 Quick4 chambers
(54 chambers total, 648 ft2 total STA). Additionally, photographs depicted that inspection ports were present at each
end of each trench.
As required by Garfield County OWTS Regulation Section 43.4.F.3, based on our final inspection observations, this
OWTS was installed according to the permit requirements and regulations. This final acceptance letter for the
installed OWTS also serves as notice that the Notice of Violation is RESOLVED.
