RE: 6800 CR 252 Garfield County, On-Site Wastewater Treatment System Construction
Certification
Dear GCPHD:
In accordance with Section 4.F of CDPHE/WQCC, Regulation #43, please accept this letter as
certification by me, the design engineer, that the subject Onsite Wastewater Treatment System
(OSWTS) was constructed in accordance with CDPHE/WQCC, Regulation #43 requirements based on
an inspection by me, November 17, 2025. It is our understanding that GCPHD also inspected the
system November 24, 2025 and required that 1) an effluent filter in the septic tank and 2) flow equalizers
(speed levelers) be installed in the distribution box. Evidence that this was completed is presented in
the installation photos included as Attachment B. The as-built configuration based on GPS survey is
depicted by the as-built/record drawing included as Attachment A. To the best of my knowledge and
measurement all offset distances have been met.
