On November 24, 2025, we visited the site of the building under construction at 6800 County Road 252
near Rifle in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit #
SEPT-8345. The system was designed by Headwater Engineering and Consulting and installed by the
homeowner. We received an as-built drawing of the constructed system and a letter certifying that the
system was constructed in compliance with the permitted design from the OWTS designer dated
December 26, 2025.
During our inspections, we observed the new cleanouts adjacent to the building, the new poly 1,000-
gallon two-compartment septic tank with an effluent filter in the outlet tee of the second compartment of
the tank, the new effluent line, the new distribution box, and the Soil Treatment Area (STA).The septic
tank and distribution box were installed level with access brought to grade. The STA consisted of four
gravelless chamber trenches. Moving from East to West the trenches contain 9, 11, 10 and 10 Infiltrator
Arc 36 chambers respectively (40 total chambers, 600 ft2 total STA). Additionally, inspection ports were
present at the ends of each trench. On December 18, 2025, we received confirmation that flow equalizers
and an effluent filter had been installed.
As required by Garfield County OWTS Regulation Section 43.4.F.3, based on our final inspection
observations, this OWTS was installed according to the permit requirements and regulations.
