Alpine Excavation and Land Clearing installed the system. The OWTS design is based on 4-bedrooms. An average daily wastewater flow of 525 GPD was used. A PVC liner was placed in the ditch within 50-feet of any OWTS component. The new, 4-inch diameter Schedule sewer line was placed as close to the foundation as possible. A section of cast iron sewer line remains under the garage apron extending into the house. The new sewer line has a double-sweep clean out and a 2% grade to the septic tank. The system installation included a 1500-gallon, two-compartment Infiltrator® poly septic tank with an Orenco® Biotube effluent filter on the outlet tee. This filter must be cleaned annually, or as needed. Risers bring the manhole lids to grade for access. A new, 4-inch diameter SDR-35 sewer line was installed between the septic tank and new distribution box down the steep hillside. The sewer line was encased in 6-inch diameter Schedule 40 pipe 10-feet on each side of the ditch crossing. A double-sweep clean out was placed on the 4-inch SDR-35 sewer line after the ditch. Effluent gravity flows to a distribution box, and then to two gravelless chamber beds. The distribution box was accessible from grade with flow equalizers installed on each outlet pipe in the distribution box to assure equal flow to each bed. A 4-inch diameter SDR-35 level manifold was installed to connect each row of the chamber beds. The chamber beds consist of 3 rows of 11 ‘Quick 4’ Low Profile Plus Standard Infiltrator® chambers for a total of 66 chambers and 792 square feet of infiltrative area. There was 6-feet of undisturbed soil between the beds. Inspection ports were installed at each corner of the beds. Ports may be cut to grade and placed in sprinkler boxes for access. Irrigation directly over the soil treatment area (STA) should be minimized. . The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes to the design and permit must be approved by Garfield County Public Health and the designer prior to construction. The new OWTS will serve a proposed four-bedroom residence. An additional profile pit and soil evaluation must be completed prior to installation of any OWTS components. The new OWTS components will consist of a new 1,500-gallon two compartment septic tank on the CDPHE approved list, set level with access to grade and an effluent filter installed in the outlet tee of the tank. The effluent line must be encased for the duration of the lined ditch crossing and for 10 feet on either side of the ditch. Effluent will gravity flow to a new distribution box set level with access to grade which will distribute effluent to 792 ft2 of STA via two level manifolds. The new STA will consist of two gravelless chamber beds consisting of 3 rows of 11 ‘Quick 4’ chambers per bed for a total of 66 chambers. Inspection ports will be installed at each corner of each bed. Maintain all setbacks and elevations as designed. The existing septic tank must be abandoned by crushing or removal. To assist in scheduling, please contact Garfield County Public Health (970-945-6614 ext. 8150) to notify the start of OWTS construction. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. NO OWTS COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR APPROVAL FROM GARFIELD COUNTY PUBLIC HEALTH AND THE DESIGNER. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction, and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection, and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced.
