Onsite Wastewater Treatment System (OWTS) Installation Observations

CBO Inc. observed the installation of the onsite wastewater treatment system (OWTS) on November 20,
2024 for the subject property. Finishing Touch Dirtworx installed the system.
The existing septic tanks were abandoned by pumping the tanks and removing them. The existing
seepage pits were abandoned by filling voids with road base. This work was completed by the original
excavation contractor, Tally Ho.
Two commercial office buildings (referenced as Building #1 and Building #2) were constructed. Each
building has four office spaces with a toilet and lavatory. With the potential for up to two 8-hour office
employees per space, we sized the OWTS to accommodate up to sixteen 8-hour employees.
An average daily wastewater flow of 240 GPD is estimated based on the anticipated number of
employees, but the OWTS design was sized on a more conservative design flow of 350 GPD.
New, 4-inch diameter Schedule 40 sewer lines were installed from each building with a double-sweep
clean outs and minimum 2% grade to the septic tank. Additional clean outs were added where the sewer
line exceeded 100-feet in length.
The system installation included a 1000-gallon, two-compartment Valley Precast concrete septic tank
with an Orenco® Biotube Pump Vault and an Orenco® PF3005 pump in the second compartment of the
septic tank. The floats were set to dose approximately 74 gallons each pump cycle. The control panel for
the pump was located within line of sight of the septic tank. The pumping system and alarm were tested
from the floats and operated normally.
Effluent is pressure dosed to through 1.5-inch diameter pump line to the level, 1.5-inch diameter manifold.
The over-excavated, unlined sand filter was 12’ x 37’. A minimum of 2-feet of sand filter material was
installed in the over-excavated footprint. A gradation of the sand media is enclosed.
Laterals used to disperse the effluent were surrounded by washed coarse screened gravel or crushed stone
(minimum 6-inches below and minimum 2-inches above laterals). Laterals were 1.5-inches in diameter with
5/32-inch diameter orifices facing down, spaced 3-feet on center (OC). Laterals begin 2-feet from the edges
of the filter, with 4-feet between the middle and outer laterals. Each 1.5-inch diameter lateral ends in a
sweeping ell facing up with a ball valve for flushing. A soil separation fabric was placed over the gravel
layer followed by approximately 1-foot of topsoil or other suitable soil able to support vegetative growth.
Inspection ports were placed in each corner of the bed.
LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must
have documented and demonstrated knowledge of the requirements and regulations of the county in
which they are working. The quality of the installation is dependent on the expertise of the installer, soil
type, and weather conditions.

Permit Conditions:
This OWTS Permit is approved on the condition of compliance with the report and design as
submitted by CBO dated February 22, 2023 and revised June 15, 2023. Changes to the design and
permit must be approved by Garfield County Public Health and the designer prior to construction.
The new OWTS will serve two new office buildings with up to sixteen 8-hour employees total. The
existing buildings are to be demolished. The existing OTWS will be removed and or abandoned in
place. The new system will consist of a new, two-compartment 1,000-gallon septic tank set level
with access to grade followed by a pump chamber set level with access to grade and a high-head
pump in the pump chamber. Effluent will be pumped to an automatic distributing valve set at the
high point of the system that will distribute effluent to 438 ft 2 of STA. The new STA will consist of an
unlined sand filter, consisting of 2 feet of sand and three laterals to be covered in washed gravel, 12-
feet wide by 37 feet long. Distribution laterals are to be 1 ½ inch Schedule 40 PVC, with 5/32-inch
orifices 3 feet on center and the laterals ending in flushing valves at the distal end of the laterals.
Inspection ports are to be installed at each corner of the bed. Maintain all setbacks and elevations
as designed.
Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final
inspection of the installed system prior to backfill.
Agreement
All installations must comply with all requirements of the Garfield County OWTS regulations
including but not limited to the conditions specified above.
In consideration of the issuance of the permit, the applicant hereby agrees to comply with all
laws and regulations related to the zoning, location, construction, and erection of the proposed
OWTS and associated structures for which this permit is granted. The applicant further agrees
that if the above said regulations are not fully complied with in the zoning, location, erection,
and construction of the above described OWTS and structure, the permit may then be revoked
by notice from the Health Department and immediately become null and void.
The issuance of a permit and specifications of terms and conditions therein will not constitute
assumption of liability, nor create presumption that the local public health agency or its
employees may be liable for the failure or malfunctioning of any system. Permit issuance will
not constitute a certification that the system, the equipment used in the system, or any
component used for system operation will ensure continuous compliance with the provision of
the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit.
Permit Approval
Plans and specifications for the proposed OWTS have been reviewed and are considered
satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated
in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from
the date of issuance unless construction on the system has commenced. An "as-built" drawing
must be submitted and inspection of the system must be completed by GCPH before final
approval of the system will be issued.
