On July 16, 2025, we visited the residence at 4361 Grass Mesa Road near Rifle in Garfield County,
Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-8947. The permit confirmed
that the existing system was sized appropriately to allow the construction of a third bedroom in the
residence. We received an inspection report certifying that the system was sized appropriately from the
OWTS inspector, Porter Project Management on October 8, 2024.
During our site visit, we did not observe obvious evidence of failure of the existing OWTS. As required by
Garfield County OWTS Regulation Section 43.4.F.3, based on our final inspection observations, this
OWTS was installed according to the permit requirements and regulations.
