The following insulation was installed by Mountain Peak Insulation LLCP
R-49 Loose fill insulation to all truss ceiling at the house with perimeter baffles
R-49 hybrid system to attic over furnace (4.5 inches of foam) and R-19 unfaced batts
R-21 unfaced batt insulation to all exterior walls
R-11 unfaced batt insulation to all 2x4 interior walls
R-19 unfaced batt insulation to all 2x6 interior walls
R-19 unfaced batt insulation to all mid floor
R-21 unfaced batt insulation to all garage common walls
R-38 loose insulation to garage with perimeter baffles
R-21 garage exterior walls with 6 mil vapor barrier
R-19 roll insulation to crawl space walls mechanically fasten
6 mil vapor barrier to crawl space floor glued at stem walls and in between seems
Fire foam penetrations, doors, windows, caulking stud packs top and bottom plates
Declarations: I hereby certify that the above insulation was installed in the building at the above location in
Conformance with the current Energy Efficiency standards for residential building Energy Code
