Conducted for:

Prepared by:

(CERT# R25-0383-AI-0) — Expires 3.14.26
(CDPHE #28789) Expires 04.24.26
Helpers Disaster Restoration LLC
11 Duroux Lane Basalt, CO 81621
(ACF # 21564) Expires 12.11.25
Background:
Helpers Disaster Restoration LLC has performed limited bulk sampling for asbestos on materials that are
planned to be removed/ disturbed during a renovation. Materials were sampled to confirm or deny
asbestos content. The planned remodel encompasses several areas of the household. It was stated that
the kitchen, fireplace, all bathrooms, the foyer, and the exterior stone wall work were going to be
remodeled. Materials were sampled using methods described in Section N.C. of the CDPHE Regulation 8.
The materials tested were in a non-invasive manner of all easily accessible materials. If materials beyond
the scope of this inspection are discovered, those materials should be considered ACMs per Colorado's
regulations and be tested by a certified building inspector to prove otherwise.
Date of Inspection:
12/05/2025 — 12/06/2025
Date of Report:
12/13/2025
Field Procedures & Analysis Methods:
Guidelines used for the inspection were established by the Environmental Protection Agency (EPA) in the
Guidance for Controlling Asbestos Containing Materials in Buildings, Office of Pesticides and Toxic
Substances, DOC #560/5-85-024 and 40 CFR Part 763, Asbestos Hazard Emergency Response Act
(AHERA). Field information was organized as per the AHERA concept of Homogeneous Area. A
Homogeneous Area is defined as a suspect material of similar age, appearance, function, and texture.
Each material was grouped together as a specific Homogeneous Area, sampled, and then assessed for
condition. Bulk samples were collected in a statistically random manner, described in the EPA "Pink
Book" (560-5-85-30A). Bulk samples of suspect ACM were analyzed by Polarized Light Microscopy
(PLM).
Homogenous Area Types & Sampling Requirements:
Surfacing Material:
Less than 1,000 square feet — minimum 3 samples to be collected Between 1, 000 — 5,000 square feet —
minimum of 5 samples to be collected Greater than 5,000 square feet — minimum of 7 samples
Miscellaneous Material (Misc):
A minimum of two samples of each homogenous area of miscellaneous material must be collected
Thermal System Insulation (TSI):
A minimum of 3 bulk samples are to be collected per homogenous area of TSI, at least one sample from
each homogenous area of patched TSI that is less than 6 square or linear feet, and in a manner sufficient
to determine whether the material is ACM, collect bulk samples from mechanical system fittings such as
tees, elbow, or valves.
Collected Assessment Categories:
1. Damaged or significantly damaged Thermal System Insulation (TSI) ACM
2. Damaged friable surfacing ACM
3. Significantly damaged friable Surfacing ACM
4. Damaged or significantly damaged friable Miscellaneous ACM
5. ACM with potential for damage
6. ACM with potential for significant damage
7. Friable ACM with low potential for damage
Other- ACM not classified in the above Assessment Categories
