AS-BUILT CERTIFICATE OF INSPECTION & ACCEPTANCE
Onsite Wastewater Treatment System

The referenced onsite wastewater treatment system (OWTS) was inspected March 4, 2026. At
the time of inspection, the OWTS was installed in general conformance with OWTS design plans
prepared July 2025.
The as-built survey was completed using Real Time Kinematic (RTK) survey equipment to
ensure accurate location and documentation of installed system components. The OWTS was
determined to be operating for its intended purpose.
