Box 1501 Glenwood Springs, CO 81601 Via email: psteely@gmail.com carla.ostberg@gmail.com jimmythebullcustomservices@gmail.com Subject: Final Acceptance Letter 175 Dolores Circle Glenwood Springs, CO 81601 OWTS Permit #: SEPT-9683 March 12, 2026 On March 10, 2026, we visited the site of the OWTS repair at 175 Dolores Circle near Glenwood Springs in Garfield County, Colorado to observe the OWTS installation pursuant to OWTS Permit # SEPT-9683. The system repair was designed by CBO Inc. and installed by Condon Civil Construction Inc. We received an as-built drawing of the existing and replaced components of the system and a letter certifying that the system was constructed in compliance with the permitted design from the OWTS designer dated March 10, 2026. During our inspection, we observed the new sewer line and cleanout adjacent to the building, the new poly 1,500-gallon two-compartment septic tank with an effluent filter in the outlet tee of the second compartment of the tank, the new effluent line, the new distribution box, and the connection to the existing Soil Treatment Area (STA). The septic tank and distribution box were installed level with access brought to grade. The STA was inspected and determined to be in good condition and sufficient for 4- bedrooms. As required by Garfield County OWTS Regulation Section 43.4.F.3, based on our final inspection observations, this OWTS repair was installed according to the permit requirements and regulations.
