Box 1534 Eagle, CO 81631 altitudeseptic@gmail.com www.altitudeseptic.com Septic System Inspection Report Property Address: 91 Michels Circle, No Name, Colorado Inspection Date: October 16, 2025 – Revisions made March 27, 2026 Inspector: Altitude Septic System Description: • Septic Tank: 1,250-gallon concrete tank 2 compartment • Risers: Inlet riser: 3 feet to the surface Outlet riser: 30 inches to the surface • Inlet Line: Cast iron • Outlet Line: Cast iron Inspection Findings: During inspection, the septic tank was found to be failing due to a visible leak along the mid-seam of the tank. The tank contained only approximately 2 feet of liquid and one foot of solids which indicates that effluent is escaping through the crack rather than maintaining proper operating levels. Both inlet and outlet sewer lines were video inspected: • Inlet Line: Constructed of cast iron. The line appears functional and intact at this time. • Outlet Line: Also constructed of cast iron but found to be in very poor condition, with several sections partially filled with dirt and debris, suggesting possible breaks or infiltration. The leach field area could not be located or accessed during this inspection; therefore, its condition could not be verified. -On 3/27/26, we were able to locate a drywell. Drywell appeared to be in good condition, but was dry due to the line being broken. System Condition: • Tank: Failed — leaking from mid-seam. • Inlet Line: Fair condition. • Outlet Line: Poor condition — partially collapsed or obstructed. • Leach Field: Unknown — unable to access or evaluate. -Went back 3/27/26 and located a drywell. Pipe is broken. Recommendations: 1. Replace the septic tank due to structural failure and leakage. 2. Replace or repair the outlet line, as it is compromised and allowing dirt intrusion. 3. Locate and evaluate the leach field to determine its current condition and functionality. – 3/27/26 Repair line to drywell 4. Install new risers and lids as needed once system repairs or replacements are completed. Overall System Status: System Fails — Septic tank leaking at mid-seam and outlet line in poor condition.
