The system was designed by Black Canyon Engineers and installed by Smith Mountain Builders. The system had been backfilled prior to a final inspection by Garfield County Public Health (GCPH), in violation of Section 43.4.E.1 of the Garfield County OWTS Regulation. As such, GCPH issued a Notice of Violation to you, the property owner, on March 9, 2026. We received an as-built drawing of the constructed system and a letter certifying that the system was constructed in compliance with the permitted design from the OWTS designer dated August 13, 2025. During our inspection and via photographs submitted by the designer and contractor, we observed the new sewer line and cleanout adjacent to the residence, the new concrete 1,500-gallon two-compartment septic tank, the distribution box, and the Soil Treatment Area (STA). The septic tank and distribution box were installed level with access brought to grade. The STA consisted of three gravelless chamber trenches, consisting of 24 Quick4 chambers per trench (72 chambers total, 864 ft2 total STA). Additionally, inspection ports were present at each end of each trench. As required by Garfield County OWTS Regulation Section 43.4.F.3, based on our final inspection observations, this OWTS was installed according to the permit requirements and regulations. This final acceptance letter of the installed OWTS also RESOLVES the Notice of Violation issued to you, the property owner, on March 9, 2026. Thank you, Edward R. White, P.E.
