Documents dated February 24, 2026 should be followed. In this letter, we describe verification of the soils, locate the water line, and clarify the depth of the required liner on the south side of the soil treatment area (STA). The 0.152-acre property is located outside of New Castle, in an area where OWTSs are necessary. Legal Description: Section: 25 Township: 5 Range: 91 Subdivision: ELK CREEK DEV. Block: 3 Lot: 9 UNIT 1 Parcel ID: 2125-251-02-009 WATERLINE LOCATION The residence is served by a community water system. The water meter is indicated on the survey in the northwest comer of the property. As shown, the water meter or curb stop is approximately 36-feet from the proposed STA. The water line runs to the east and enters the northern portion wall of the residence. A minimum 25-foot separation from the proposed STA to the waterline must be maintained. SUBSURFACE The subsurface was investigated on April 10, 2026 by digging one soil profile test pit excavation (Test Pit). Only one Test Pit was excavated due to the limited space available on the property. A visual and tactile soil analysis was completed by Carla Ostberg at the time of excavation.' The materials encountered in Test Pit #1 consisted of medium loamy topsoil and rootzone to 3.0-feet, underlain by medium brown loam to a maximum depth explored of 6.0-feet. No bedrock or groundwater was encountered. The existing STA (chambers) was encountered at approximately 5.0-feet below native grade. There was evidence of sludge in the chambers, but because the house has been vacant for several months, the soils were not saturated. A sample of the soil was taken from Test Pit #1 at 3-feet below grade. Soil structure grade was moderate. The soil formed a ball and a ribbon approximately 1-inch in length before breaking. Soil structure shape was blocky and consistence was friable. Soil texture was both gritty and smooth. Page 2 The soil is consistent with Soil Type 2, Loam, with moderate structure grade. A long-term acceptance rate (LTAR) of 0.6 gallons per square foot was used to design the STA. This sizing is appropriate based on verification of the soil. The 0.152-acre property is located outside of New Castle, in an area where OWTSs are necessary. Legal Description: Section: 25 Township: 5 Range: 91 Subdivision: ELK CREEK DEV. Block: 3 Lot: 9 UNIT 1 Parcel ID: 2125-251-02-009 SITE CONDITIONS The property is currently developed with a 3-bedroom residence. The residence is served by an existing OWTS which was inspected by Voskuil Operations on December 29, 2025. The OWTS consists of a 1000-gallon, two -compartment Copeland Concrete septic tank followed by a soil treatment area (STA) with gravelless chambers (permit is difficult to read). The system was sized to accommodate 3 bedrooms. Garfield County Individual Sewage Disposal Permit 2189 documents the system. The permit received final approval on December 1, 1993. The inspection report is enclosed with recommendations, including replacing the STA due to failing the hydraulic load test. The residence is served by a community water system. The water meter is indicated on the survey in the northwest corner of the property. As shown, the water meter or curb stop is approximately 36-feet from the proposed STA. The water line should be located as part of the layout of the proposed STA to assure the 25-foot setback is maintained. The manufactured home has been placed on footers and stacked cinder blocks. The proposed STA is approximately 13.5-feet from the manufactured home. A retaining wall has been constructed along the southern portion of the property. A 30 mil PVC liner will be installed on the southern portion of the proposed STA to prevent lateral migration of effluent towards the retaining wall. The proposed soil treatment area (STA) location is vegetated with grasses as part of the yard area. Page 2 There should be no traffic or staging of material over the future STA site to avoid compaction of soils prior to construction of the STA. SUBSURFACE To minimize disturbance in the area of the proposed STA, we have utilized previous soil investigations and other resources to assess soils for sizing the STA. A Test Pit must be excavated prior to installation of the STA. This office must be called to verify soils are consistent with what has been outlined in this design. If soils are not consistent with our assumptions, design changes may be necessary. We have researched soils in the area through USGS Web Soil Survey. The report is enclosed. The area of the proposed STA is identified by map unit symbol 58. The map unit name is Potts-Ildefonso complex, 12 to 25% slopes. Typical soil profile from 0 to 4-inches consist of loam, 4 to 28-inches consist of clay loam, and 28 to 60-inches consist of loam. Garfield County ISDS Permit 2189 documents the percolation rate of 27 to 30 minutes per inch (MPI). Based on information provided from the available documented resources and our knowledge of soils in the area, we are assuming soils to be consistent with Soil Type 2. A long-term acceptance rate (LTAR) of 0.6 gallons per square foot will be used to design the STA. Soils must be verified by this office prior to construction of the STA. Photos provided document soils on a property (112 Navajo Street, Parcel ID: 2125-251-02-018) located approximately 200-feet south of the subject property. A seepage pit was installed, with excavation extending approximately 10-feet below native grade. Soils were consistent with light brown loam to sandy loam with minimal rock content. An average daily wastewater flow of 450 GPD will be used. For the purposes of this OWTS design, Benchmark Elevation has been established as 100' (existing sewer line exiting residence). CBO Inc. should be notified of any discrepancies or problems with grade elevations of proposed components during installation of the OWTS. Component elevations may change during installation due to site conditions. Minimum grade refers to piping between components. The existing 3-inch diameter sewer line under the residence should be corrected to eliminate the belly and achieve the correct slope. This sewer line will tie into the existing 4-inch sewer line. A double -sweep clean out must be installed on the sewer line between the residence and the septic tank. The sewer line must have a minimum 2% grade to the septic tank. The existing septic tank will remain. The manhole lids must be repaired and/or replaced. An Orenco® Biotube effluent filter must be installed on the outlet tee. The existing STA will be abandoned in place. If saturated soils are encountered, the soils should be excavated and staged for drying prior to disposal. If over -excavation is necessary in the proposed STA, soils may be replaced with concrete (washed) sand. Sand filter material must be clean, coarse sand, all passing a screen having four meshes to the inch. The sand must have an effective size between 0.15 and 0.60 mm. The uniformity coefficient must be 7.0 or less. Material meeting ASTM 33, for concrete sand, with three percent or less fines passing 200 mesh sieve may be used. If needed, a gradation of the sand media must be submitted to this office prior to obtaining the sand. Effluent will gravity flow to a distribution box, and then to two gravelless chamber beds. The distribution box must be accessible from grade and must have flow equalizers, or similar product, installed on each outlet pipe in the distribution box to assure equal flow to each bed. Each chamber bed will consist of 4 rows of 7 'Quick 4' Infiltrator@ Low Profile chambers for a total of 56 chambers and 672 square feet of infiltrative area. A 4-inch diameter SDR-35 level manifold must be installed to connect two rows in each bed (see W3.1 Chamber Bed Plan). Inspection ports must be installed at each corner of the beds. Ports may be cut to grade and placed in sprinkler boxes for access. A 30 mil PVC liner must be installed on the downhill (south side) of both beds. The installer should work with this office to determine the best placement of the liner to protect from lateral migration of effluent. Page 5 COMPONENT SPECIFICATIONS The component manufacturers are typical of applications used by contractors and engineers in this area. CBO Inc. must approve alternative components prior to installation of the OWTS. Requests must be submitted, in writing, to our office for approval prior to installation. VALIDITY PERIOD OF DESIGN DOCUMENTS This design is valid for submission of an application for permitting for l.vvo (2) years from the date of the design packet or upon a change in the county OWTS Regulation that could impact compliance with code requirements. Continued use beyond this period or following a change in the regulation is not recommended without review and update of design documents. INSTALLATION CONTRACTOR CBO Inc. expects that the installer be experienced and qualified to perform the scope of work outlined in this design. The installer must review this design thoroughly and coordinate with our office in advance of installation. Any additional conditions in this design or county permit must be completed and documented prior to final approval of the OWTS installation. Communication between the installer and this office is expected throughout the installation. INSTALLATION OBSERVATIONS CBO Inc. must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. CBO Inc. should be notified 48 hours in advance to observe the installation. In an effort to improve the accuracy of the record drawing, we request that the installer provide a sketch of the installation, including path of the sewer lines, water line installation (if applicable), septic tank location, STA location, and measurements from building corners or another fixed objects on the property. This sketch is most easily provided on Sheet W2.0 of the OWTS Design Page 6 Packet. Photographs of the installation and final cover are also requested to supplement our installation documentation. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irrigation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo-fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on the backfilled STA. Machines with tracks (not wheels) should be used during construction of the STA for better weight distribution. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be flushed annually, or as needed. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high- water conditions and shall be connected to a control breaker separate from the high-water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. Excavation equipment must not drive in the excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3-inch. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in this report, CBO Inc. should be notified. All OWTS construction must be according to the county regulations. Requirements not specified in this report must follow applicable county regulations. The contractor should have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The three -bedroom residence is currently occupied. A two-way exterior cleanout was located in the front yard off the porch (southeast side of residence) just downstream from the septic tank. There is a drain grate as the cap for the cleanout. - There is no exterior cleanout present between the residence and the septic tank and we were unable to inspect this portion of the sewer line. The septic tank is located in the front yard on the southeast side of the residence. Garfield County records show final inspection of the tank and system components in December 1993 (see additional documents at end of report). Both concrete lids were at surface grade. The concrete tank is a dual compartment style with 2 concrete lids for access. The lid on the outlet side of the septic tank is broken and cracked. The handle on the lid of the inlet side of septic tank is rusted out of the concrete. The approximate tank capacity is 1000 gallons. The concrete risers were probed and found solid at time of inspection. The inlet pipe is located under the top of the tank. ~ ' E . 'i-. •c •. �,'��� , . _. k�:.u, �'' � ti.' - ~ti , b We video inspected from the cleanout under the residence downstream approximately 50 feet, but unable to round the corner where the sewer line exits from under the residence. The pipe consists of 3" ABS sewer pipe under the residence before transitioning to 4" sewer pipe as it leaves the residence. There is a belly in the 3" section of ABS sewer line under the residence just downstream from the cleanout. We video inspected from the exterior cleanout located downstream of the septic to the manifold until we could not push the camera to continue the inspection due to several bends. The discharge pipe consists of 4" PVC pipe to the manifold and STA (soil treatment area). Hydraulic Load Test: Using a garden hose, we applied 135 gallons of water at a rate of 9 gallons per minute down the exterior cleanout on the discharge side of septic tank. Water was back flowing into the septic tank outlet at 15 minutes and STA was not accepting flow. Recommendations: - Because the soil treatment area did not pass the hydraulic load test, it is highly recommended that the soil treatment area be replaced. Pump septic tank every 3 - 5 years depending on use and prior to transfer of title. The existing tank is adequate for current design criteria according to Garfield County OWTS regulations. For a 3-bedroom residence, the septic tank must be at least 1000 gallons, have two compartments with x2 accessible lids at the surface. Replace the broken concrete lid on outlet side. Install a new stainless -steel handle on the concrete lid on the inlet side. - It is recommended to include an effluent bio filter on the outlet tee of the septic tank. The bio filter should be inspected and cleaned depending on use (refer to manufacture recommendations, or more often if use patterns indicate). Correct the slope (due to belly) in the 3" ABS pipe under the residence. Install an exterior cleanout between the residence and the septic tank. Based on what we were able to observe and our experience with onsite wastewater technology, we submit this Onsite Wastewater Treatment System Inspection Report based on the present condition of the OWTS. Voskuil Operations has not been retained to warrant, guarantee, or certify the proper functioning of the entire system for any period of time in the future. Because of the numerous factors (usage, soil characteristics, previous failures, etc.) which may affect the proper operation of a wastewater treatment system, this report shall not be construed as a warranty by our company that the entire system will function properly for any particular buyer, seller or property owner.
