Voskuil Operations, LLC installed the system. The OWTS design is based on 3-bedrooms. An average daily wastewater flow of 450 GPD was used. The waterline was located onsite and is approximated on the record drawing. The waterline is greater than 25-feet from the soil treatment area (STA). The existing 3-inch diameter sewer line under the residence was corrected to eliminate the belly and achieve the correct slope. This sewer line tied into the existing 4-inch sewer line. A double-sweep clean out was installed on the sewer line between the residence and the septic tank. The sewer line had a minimum 2% grade to the septic tank. The existing septic tank remained. The manhole lids were replaced. An Orenco® Biotube effluent filter was installed on the outlet tee. This filter must be cleaned annually, or as needed. The existing STA was abandoned in place. Effluent gravity flows to a distribution box, and then to two gravelless chamber beds. The distribution box was accessible from grade with flow equalizers installed on each outlet pipe in the distribution box to assure equal flow to each bed. Each chamber bed consists of 4 rows of 7 ‘Quick 4’ Infiltrator® Low Profile chambers for a total of 56 chambers and 672 square feet of infiltrative area. A 4-inch diameter SDR-35 level manifold was installed to connect two rows in each bed. Inspection ports were installed at each corner of the beds. Ports may be cut to grade and placed in sprinkler boxes for access. A minimum 30 mil PVC liner was installed on the downhill (south side) of both beds. The depth of the liner extended to the bottom of the upper tier of the retaining wall. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. CBO Inc. should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Changes to the design and permit must be approved by Garfield County Public Health prior to construction. The new OWTS will serve an existing three-bedroom residence. The slope of the existing 3-inch sewer line under the residence must be corrected to remove a belly. A cleanout must be installed in the existing sewer line outside the residence. All existing sewer line to remain in use must be found in good working order. An existing 1000-gallon two compartment septic tank set level with access to grade has been found to be in good working order and will remain in use. An effluent filter must be installed in the outlet of the septic tank, and the manhole lids must be repaired or replaced. Effluent will gravity flow to a new distribution box set level with access to grade which will distribute effluent to 672 ft2 of STA. The new STA area will consist of two gravel-less chamber beds. Each bed will consist of four rows of 7 ‘Quick 4’ chambers (56 chambers total). Should the existing STA being encountered during construction, the infiltrative surface must be found in good condition and able to accept effluent and any sludge and saturated soils must be removed and replaced with sand filter material. To prevent lateral surfacing of effluent, a 30 mil PVC liner must be installed on the Southern side of both beds to the depth of the bottom of the first tier in the retaining wall to the South. Inspection ports will be installed at each corner of each bed. Maintain all setbacks and elevations as designed. Contact the designer and Garfield County Public Health at least 48 hours prior to requesting final inspection of the installed system prior to backfill. NO OWTS COMPONENT SHALL BE BACKFILLED WITHOUT PRIOR APPROVAL FROM GARFIELD COUNTY PUBLIC HEALTH AND THE DESIGNER. Agreement All installations must comply with all requirements of the Garfield County OWTS regulations including but not limited to the conditions specified above. In consideration of the issuance of the permit, the applicant hereby agrees to comply with all laws and regulations related to the zoning, location, construction, and erection of the proposed OWTS and associated structures for which this permit is granted. The applicant further agrees that if the above said regulations are not fully complied with in the zoning, location, erection, and construction of the above described OWTS and structure, the permit may then be revoked by notice from the Health Department and immediately become null and void. The issuance of a permit and specifications of terms and conditions therein will not constitute assumption of liability, nor create presumption that the local public health agency or its employees may be liable for the failure or malfunctioning of any system. Permit issuance will not constitute a certification that the system, the equipment used in the system, or any component used for system operation will ensure continuous compliance with the provision of the OWTS Act, the regulations adopted thereunder, or any terms and conditions of a permit. Permit Approval Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby given to the property owner(s) to perform the work indicated in accordance with the Garfield County OWTS Regulation. This Permit will expire 1 year from the date of issuance unless construction on the system has commenced.
