On June 30, 2025, GCPH issued Vault and Haul OWTS Permit # SEPT-9323. The Permit is valid until
June 30, 2026. During vault and haul operations, the site temporarily housed up to a maximum of eight
individuals during natural gas drilling activities.
On November 5, 2025, GCPH received notification that vault and haul operations have ceased and all
associated equipment has been removed from the site. As required by Garfield County OWTS Regulation
Section 43.4.F.3, based on our final inspection observations, this OWTS was installed according to the
permit requirements and regulations.
