'CONDITIONS:
1. All installation must comply with all requirements of the County Individual Sewage Disposal Regulations, adopted pursuant to aui
thority granted in 66 -44.4, CRS 1963, amended 66-3-14, CRS 1963.
2. This permit is valid only for connection to structures which have fully complied with County Zoning and building requirements.
Connection to or use with any dwelling or structures not approved by the building and Zoning office shall automatically be a viola -
tion of a requirement of the permit and cause for both legal action and revocation of the permit.
3. Section III, 3.24 requires any person who constructs, alters, or installs an individual sewage disposal system in a manner which in-
volves a knowing and material variation from the terms or specifications contained in the application of permit commits a Class 1,
Petty Offense ($500.00 fine — 6 months in jail or both.

As per our conversation this date, I am forwarding to you two (2) copies of
the Individual Sewage Disposal Permit 8491 issued to Lot 11, Filing 2, of West -
bank Subdivision.
This documentation will verify the individual sewage disposal system serving
the residence on the property is in compliance with present Garfield County and
Colorado State Regulations.

Dear Mr. Kerst:
As per our telephone conversation April 12, 1t78, I am forwarding to you
a copy of the Garfield County Individual Sewage Disposal Permit #491 which was
issued to Mr. Maynard Nelson and David C. Moore. As you will note, a final
approval of the system was given February 3, 1978. It has now come to our
attention, as you know, that a septic tank rather than an aerated treatment unit
was installed on the property. For this reason, the system presently in existence
does not completely comply with the requirements of the permit. This letter will
also serve to inform you that this Department has no objections to your proposal
of placing monies in escrow to cover possible future construction costs of replac-
ing the septic tank with an aerated treatment unit and bringing the individual
sewage disposal system for Lot 11, Filing 2, Westbank Subdivision into compliance
with the Garfield County Individual Sewage Disposal Systems Regulations and Zoning
Resolution.
