Enclosed is your copy of the above report form for the inspection perionncd on July 12, 1995. During the inspection
the following items were noted and need to be brought to your attention:

2. The travel trailers were discharging grey water onto the surface of the ground and into an adjacent inigation
ditch.
This is to notify you that the conditions at your Industtal Wastewater Treatment Facility as listed above constitute a
violation of the Colorado Water Quality Control Act. Please respond within 30 days of receipt of this letter with your
plans to bring this wastewater treatment system into compliance. Failure to notify this office within the required time
limit may result in legal action.

Dear K. R.
SGM has completed the design for the septic systems for the maintenance and office buildings
at the Sievers Pit site. This design is based upon percolation data collected by SGM through
percolation testing during the week of April 19.
The results of those percolation tests indicated that the proposed disposal fields for both the
office building and maintenance building are going to be located in highly pervious alluvium -type
material. Percolation rates were measured at 5 minutes per inch (mpi) or faster. Based upon
these percolation rates, a sand filter type construction for the disposal field will be required at
both sites. It is important to note that the percolation testing was done at the proposed field
location for the maintenance building. We are assuming at this point in time that the proposed
field location for the office building will have very similar soils. However, verification (through
profile hole excavation and percolation testing) will be needed prior to construction of the field
at the proposed office building site.
Maintenance Building ISDS
For the maintenance building, we are assuming that 10 persons per day will be the design
number of employees utilizing this facility. Based upon this employee count and the average
flow rate of 15 gallons per employee per day, would yield an average flow for the maintenance
building of 150 gpd. At the peak design flow, this figure would adjust up to 225 gpd. Based
upon this figure, the required septic tank capacity would be at 340 gpd. However, the minimum
size septic tank as required by the State Health Department is 750 gpd. Therefore, based upon
septic tank minimal requirements, as mandated by the State Health Department, we have also
sized the disposal field based upon these minimal requirements. We have calculated the size of
the disposal field to be 223.6 square feet. This equates to a field with dimensions of 12' x 19'.
We have attached a sketch which identifies the construction requirements (both in plan view and
section) of the disposal field. Finally, we have also attached a sketch which identifies in
schematic form the components of the ISDS.

Office Building
The design for the ISDS for the office building was based upon the observed number of offices
provided within the building as well as an assumed 10 visitors per day. In essence, this system •
would be sized for 20 persons per day. The average daily flow, based upon 20 persons per day,
would be 300 gpd. The design flow, therefore, would be 450 gpd. This would relate to a septic
tank capacity of less than 750 gpd. Again, the minimal requirements mandated by the State
health Department would control this design. Accordingly, the disposal field size for the office
building ISDS would be based upon the minimal State Health Department requirements. The
sketches, as provided and discussed above for the maintenance building, are also applicable for
the office building as well.
