CONDITIONS OF APPROVAL
1) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of the
permit does not assure the applicant that no injury will occur to another vested water right or preclude another
owner of a vested water right from seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction and Pump Installation Rules
2 CCR 402 -2, unless approval of a variance has been granted by the State Board of Examiners of Water Well
Construction and Pump Installation Contractors in accordance with Rule 17.
3) Approved pursuant to CRS 37- 92- 602(3)(b)(II)(A) as a change of use of permit no. 173635 (canceled), and as the
only well on a residential site of 19.03 acres described as lot 2, Irene Green Subdivision Exemption, Garfield
County. The issuance of this permit hereby cancels permit no 173635.
4) The use of ground water from this well is limited to ordinary household purposes inside a single family dwelling.
The ground water shall not be used for irrigation or other purposes.
5) The maximum pumping rate shall not exceed 15 GPM.
6) The return flow from the use of the well must be through an individual waste water disposal system of the
non - evaporative type where the water is returned to the same stream system In which the well is located.
7) This well shall be constructed not more than 200 feet from the location specified on this permit.

Dear Mr. and Mrs. Ochko:
As requested, Hepworth - Pawlak Geotechnical, Inc. conducted percolation testing and
observed backhoe pit excavations at the subject site on April 14, 1994. The findings of
our work are presented in this report. The work was done in accordance with our
agreement for geotechnical engineering services to you, dated April 5, 1994.
A new 1 1/2 story residence and leach field for septic disposal are proposed. The
percolation testing was conducted for design of an infiltration disposal system and
backhoe pits were dug near the residence for general bearing conditions.
Two pits were dug near the proposed residence and a profile pit was dug in the leach
field area as located by you. The soils encountered in the pits consisted of low density
sandy silt in the residence area (Fig. 1) and about 3 feet of silt above the basalt gravel
and cobbles in a sandy silt matrix Lathe leach field area (Fig 2). Results of
consolidation testing performed on samples taken from the building site, shown on
Fig. 3, indicate the silts are highly compressible when wetted and loaded. No free
water was encountered in the pits and the soils were slightly moist. Percolation testing
was performed in the basalt gravel cobbles in 3 shallow pits on April 15, 1994.
Considering the conditions exposed in the pits dug near the building site, spread
footings placed on the natural silt soils could undergo excessive settlement even under
Light loading if wetting of the subsoils were to occur. Bearing the footings on the silty
gravel and cobble subsoils should have relatively low settlement risk. Moving the
building site and /or deepening the bearing level to the gravel and cobble subsoils should
be considered to reduce the risk of building distress. The percolation test results are
presented in Table I. Based on the subsurface profile and the test results, the designated
leach field area should be suitable for an infiltration septic disposal system. The
average percolation rate measured is about 10 minutes per inch.

The findings of this study are based on our observation of the soils exposed within the
pits excavated at the designated locations. The extent of the compressible silt was not
determined in the residence area. Variations in the subsurface conditions could increase
the risk of foundation movement. Additional subsurface exploration is recommended to
determine the depth of the silt and a suitable building location.
