•CONDITIONS:
1. All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter
25, Article 10 C.R.S. 1973, Revised 1984.
2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con-
nection to or use with any dwelling or structures not approved by the Building and Zoning off ice shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which Involves a knowing and material
variation from the terms or specifications contained In the application of permit commits a Class I, Petty Offense ($500.00 fine — 6
months in Jail or both).

AGREEMENT REGARDING ISDS PERMIT
WHEREAS, Don Reinarz (hereinafter "Owner") has applied for an Individual
sewage disposal system (ISDS) for a house to be located on lot 5 in a subdivision
known as the Westhoff Subdivision, on County Road 110, Garfield County, Colorado
and,
WHEREAS, the Garfield County Building and Planning Department normally
does not issue ISDS permits without a building permit for a residential structure being
issued at the same time;and
WHEREAS, the Owner and Building & Planning Department agree that there was
a misunderstanding regarding the County's policy regarding the issuance of ISDS
permits at the same time as a building permit; and
WHEREAS, the Owner is willing to make certain assurances and agreements
related to the Issuance of an ISDS permit prior to the issuance of a building permit and
on the basis of such assurances the County is willing to issue an ISDS permit .
NOW, THEREFORE, it is agreed as follows:
A. The Owner agrees that no residential dwellings, temporary or permanent, will
be placed or built on the property known as Lot 5 of the Westhoff Subdivision,
prior to the approval of a building permit application for any such structure by the
Garfield County Building & Planning Department.
B. The Owner agrees that the approval of the ISDS permit does not entitle the
Owner to use the ISD system for any purpose, until said building permits are
approved for the use.
C. The Owner agrees that any change in plans requiring the size of the ISD
system to be enlarged will require the issuance of an additional ISDS permit, at
the fee or costs in place at the time of the issuance of the additional permit.
D. This agreement is binding upon the heirs, successors in title, and assigns of
the parties.
