2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con- ,
nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be a violation or a
requirement of the permit and cause for both legal action and revocation of the permit.
3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material
variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —6

Subject: Percolation Testing, Proposed Septic Disposal System Addition, Hardin
Residence, 2085 Road 112, Garfield County, Colorado.
Dear Mr. Hardin:
As requested, a representative of Hepworth - Pawlak Geotechnical, Inc. conducted
percolation testing at the subject site to evaluate the feasibility of additional infiltration
septic disposal area. The results of our work are presented in this report.
A profile hole and three shallow backhoe pits were excavated at the locations shown on
Fig. 1. The percolation test holes were dug in the potential expansion area surrounding
the existing leach field area. Percolation test holes were hand dug in the bottom of the
backhoe pits and soaked one day prior to testing. The test holes were covered with
insulation overnight to protect against freezing. The soils exposed in the percolation
holes were similar to those exposed in the profile boring and consist of about ' foot of
topsoil overlying relatively stiff silty sandy clay to the profile boring depth of 9 feet.
Groundwater was not encountered in the profile boring and the soils were slightly moist
to moist.
Percolation tests were performed on March 13, 2001 by a representative of Hepworth -
Pawlak Geotechnical, Inc. The percolation test results are presented on Table I. The
percolation test results indicate an infiltration rate of about 30 minutes per inch. The
variable results of P -1 were discounted due to the presence of an apparent animal hole
in the hand dug percolation test hole which did not become evident until the percolation
test was performed. Based on the subsurface conditions observed and the percolation
test results, the tested area should be suitable for a conventional infiltration septic
disposal system.

Dear Dan:
Pursuant to your request, attached herewith is a letter /report presenting our findings in regard to the design
of an Individual Sewage Disposal System (ISDS) at the above referenced Site. This design is based on our
evaluation of the site conditions with information provided by you for use in supporting your application to
the Garfield County. Our recommendations are in accordance with Garfield County and the State of
Colorado ISDS Regulations. The Garfield County must permit any proposed improvement to the site. We
have reviewed the information forwarded to us, conducted a site visit, formulated an ISDS design and
created a site plan with construction details as part of our scope of work.
Conclusions:
Based on our findings we believe that the design and installation of an approved ISDS system is feasible in
accordance with the Regulations of Garfield County and the State of Colorado. The existing septic system
is currently in use and adequately functioning. Therefore we recommend that its use continue to accept
wastewater from the existing bathrooms only. The remodeled kitchen and laundry facilities will be
plumbed to discharge to a new proposed system. We recommend that a new 1000 - gallon septic tank be
installed that will discharge effluent to an approximate 760 square foot soil absorption trench system to
serve the new addition and remodeled kitchen and laundry facilities. The new soil absorption system can
be installed in natural soils. The system will meet all required setbacks and be installed within the general
boundaries indicated on the plan. Our Design is outlined below and delineated on the attached site plan.
Site Location:
The subject site is located in the Crystal Springs area, adjacent to the south side of County Road 112,
Approximately 3 miles North of State Highway 82 in Garfield County, Colorado. The site is situated in
Section 14, T 7 S, R 88 W of the 6 P.M. The Site comprises approximately 5.25 acres. The site is
bounded on the northeast and east by a private access road and on the south, north and west by single
family acreages and south of County Road 112.
Existing Site Conditions:
The site is located on a gentle to moderate sloping pasture and rangeland. The site has been previously
subdivided out of larger improved ranch land. The site includes a 3- bedroom house with and existing
permitted ISDS system, a detached shed and barn structure. The site has a general slope toward the East.
The existing ground surface in the proposed septic development area ranges from 3% to 15 %. The

developed site and undisturbed areas are covered with grasses, scattered trees and landscaping shrubs.
Domestic water is supplied by an existing registered well in excess of 100 feet from the closest point of the
proposed absorption trenches.
Proposed Site Conditions:
It is our understanding that you intend to construct a 1200 S.F. residential addition to include 2 new
bedrooms and 2 full baths. We further understand that the existing kitchen and laundry facilities will be
redesigned and re- plumbed to discharge to the new proposed ISDS. The wastewater drainage from these
facilities will be separated from the existing ISDS. Therefore the existing system will treat the runoff
form the two existing full bathrooms only. The proposed improvements will include a new ISDS system to
serve the needs of an equivalent 5- bedroom house minus the contribution from two full baths. The new
facilities are to be generally located as shown on the plan.
Subsurface Conditions:
Attached herewith are the results of the subsurface investigation and percolation tests conducted on March
13, 2001 by Hepworth - Pawlak Geotechnical, Inc. The report is dated March 15, 2001. The subsoil
encountered at the site consists of 1/2 foot of topsoil overlying relatively stiff silty sandy clay to the profile
boring depth of 9 feet. Groundwater was not encountered to a depth of 9 feet in the profile boring.
Percolation testing utilized three test holes, which yielded similar absorption rates. The average
percolation rate is 30 minutes per inch and is suitable for a conventional absorption system.
Design Criteria:
The site will have two separate systems that combined will handle and treat the effluent from a five
bedroom home. The existing system will continue to handle and treat the wastewater from the two existing
full bathrooms only. The proposed ISDS system to serve the needs of an equivalent 5- bedroom house
minus the contribution from two full baths.
The existing home is a split -level house with the existing ISDS installed at grades that prohibit the gravity
discharge of effluent from the lower level. The proposed improvements and plumbing will be designed at
the lower level finish floor elevation. Therefore the new system will be lower than the existing and will be
hydraulically separate.

Dear Dan:
Pursuant to County Regulations, this letter and attached drawing provides documentation that the new
ISDS system recently installed is in general compliance with the permitted design. Sopris Engineering has
performed site visits to measure, inspect, and document the as built conditions of the constructed system.
We have coordinated our efforts with the contractor that built the system. The system was inspected prior
to backfilling and after all installations was completed. The as built conditions and installation of the new
ISDS components is in compliance with the permitted design specifications for the system. The as built
size and relative location of the new ISDS system is delineated on the attached drawing.
A 1000 gallon dual compartment septic tank was installed with 4" tee baffles and an effluent filter.
Inspection ports were installed on the ends of the trenches. The absorption field was constructed as
delineated on the site plan. The location of the trenches have been changed slighty by rotating and moving
approximately 6 feet to the Northwest closer to the house. The four 3' x 69' trenches were installed level
utilizing specified materials installed according to design. The minimum setback distances have been
maintained.
The septic tank, distribution pipe, and installation are in accordance with Garfield County Regulations, the
design presented in the Sopris Engineering Report and the design drawings, dated April 10, 2001.
Attached is an as -built drawing dated 8/14/01.
