All installation must comply with all requirements of the Colorado State Board of Health Individual Sewage Disposal Systems Chapter 25, Article 10 C.R.S. 1973, Revised 1984. 2. This permit is valid only for connection to structures which have fully complied with County zoning and building requirements. Con- nection to or use with any dwelling or structures not approved by the Building and Zoning office shall automatically be aviolation or a requirement of the permit and cause for both legal action and revocation of the permit. 3. Any person who constructs, alters, or installs an individual sewage disposal system in a manner which involves a knowing and material variation from the terms or specifications contained in the application of permit commits a Class I, Petty Offense ($500.00 fine —6 months in jail or both). The OWS design is based on a sewage load of 900 GPD and a percola- tion rate of 78 MPI. A 25% field size reduction was taken due to the dosing of the trenches. Vehicle movement over the drain field trench area during construction should be kept at a minimum to reduce compaction of the receiving soils. The bottom of the trenches should be scarified with a pick, shovel, or backhoe bucket teeth prior to the installation of chambers. The surface of the field should be seeded after installation of the trenches. A native seed mix available at seed stores should be sown These mixes do not require irrigation and develop a growth 10 to 15 inches high. No automatic sprinkler system should be installed over the trenches. OPERATION AND MAINTENANCE The owner must realize an OWS is different from public sewer service. The owner must be aware of, and assume responsibility for maintenance of the system. In the absence of a monitoring program, the septic tank should be pumped every two years. Effluent filters and screened vaults, as applicable, are to be cleaned during pumping. There are daily considerations such as not putting plastic or other nonbiodegradable materials into the septic system. Garbage disposal use should be minimized. The backwash of a water softener may be harmful to the OWS. A separate drywell should be constructed for the backwash waste, if a softener is installed. Chemically treated water from a swimming pool or spas should not be introduced to the OWS. LIMITATIONS An OWS design should be installed by a contractor who is experienced in its installation. Our investigation, layout, design and recommendations are based on data submitted. If subsurface conditions considerably different from those described in this report are encountered, we should be notified to evaluate the effect on the proposed OWS. If the health department makes modifications to this design, we should be contacted to evaluate the impact on the performance of the OWS. The county and our firm must be contacted to observe installation of the OWS All construction is to be in accordance with the ISDS regulations. Pipe type and size, burial requirements, septic tank construction, and other specifications, which are not depicted in our report, are to conform to the requirements of the ISDS regulations. The installer of the system is to have demonstrated knowledge of the ISDS regulations and requirements. We must be called to observe installation of the OWS. Sincerely, ...... .o� ... ENGINEERS & GEOLOGISTS December 4, 2001 Jeff LaBaw 697 Mesa Avenue Rifle, Colorado 81650 Subject: Installation Observation, Proposed Residence 4000 County Road 302 Garfield County, Colorado Job No. 14452 Mr. LaBaw, As requested, we observed the installation of the onsite wastewater system (OWS) for the subject site on November 20, 2001. The system was designed under our Job No. 14452, dated October 29, 2001. The system includes the installation of one 1250 - gallon, single - compartment, precast concrete septic tank with an effluent filter on the outlet followed by one 1000 - gallon, two - compartment, precast concrete septic tank with a dosing siphon in the second compartment and six gravelless chamber- trenches. Three trenches (A) had 1 of 16 chambers each, two trenches (B) had 1 row of 11 chambers each, and one trench (C) had 1 row of 10 chambers for a total of 80 chambers and 1240 square feet of infiltrative area, as indicted on Figure 1. Two distribution boxes were used to distribute effluent evenly throughout the field. The dosing siphon was in place at the time of our site visit. The construction foreman, Jeff LaBaw, may activate dosing system at his discretion. The components of the OWS were installed in conformance with our plans and specifications.
